As Randy points out when they're all at Thanksgiving dinner at Willy's (Frank's brother) house -- which Frank invited himself and Charlie to -- Frank was a respected officer but also an often irresponsible drunk. While partying with friends one night, Frank was juggling grenades, he pulled at least one of the pins and the grenade exploded, blinding him. Randy also points out that Frank was drunk, adding to the severity of the accident. It's not explained if the grenade damaged Frank's eyes directly or not, since they don't appear to have any noticeable damage but he has a noticeable scar on one of his temples so it's possible the force of the explosion injured his head and blinded him that way. In the end, Frank became an even more bitter and hostile person due to his injury.
