Continuity

The Chancellor's car during the final scenes lacks continuity. It starts as a 2022 Range Rover, then as it drives through the fence, it is an old Lada Niva, then during the speed sequence it is 2010 Range Rover, before, at final showdown, becoming 2004 Range Rover.
When the chancellor attempts his escape with Ella he gets into a late model Range Rover however when he crashes after running into EVE its an earlier model of the same make .

Factual errors

When Eve grabs the fire hose, she doesn't turn the water on. She just walks outside and starts spraying to fight the guy with the flame thrower.
During the cult's attack on Frank's "shop," they shoot straight through the reinforced front door to access the armory. However, when Eve escapes into the next room, she takes cover behind an internal door and appears confident she is safe. There's no indication that this interior door is more bullet-resistant than the front entrance, making her behavior inconsistent and illogical given the threat.
The Chancellor loads a single round into one of the chambers of the .38 revolver's cylinder before handing it to Eve's father. When he closes the cylinder, the loaded chamber is aligned with the barrel. But because the cylinder rotates as the hammer is cocked, the hammer will fall on an empty chamber.
A claymore does not produce a huge explosion with a fireball. It is not an explosive in the traditional sense, rather it is a directional antipersonnel mine that uses a shaped charge of C4 to propel roughly 700 steel ball bearings 1/8 inch (3.2 mm) in diameter (about the same size as #4 goose shot), at a velocity of roughly 1,000 meters per second (3,280 ft/sec), which is nearly three times the average muzzle velocity of a 12 gauge 2-3/4" shotshell loaded with #4 shot (1,200 ft/sec). When detonated the ball bearings are shot out in a 60-degree arc in front of the mine, which is usually aimed at an upward angle so the bearings hit the target(s) center-of-mass, shredding their abdominal organs into hamburger. As such a claymore doesn't rely on the actual explosion for lethality, but rather the shrapnel it propels.
The last man Eve kills with the flamethrower gets multiple shots which cover his entire body, but his face and hair don't burn.

Incorrectly regarded as goofs

After Chapter 3, John Wick is officially excommunicated and should have no ties to the Ruska Roma. Yet in this film, he is dispatched by them to deal with Eve, raising questions about why they would rely on a pariah to enforce their will.

Correction: It is because of John's excommunicado status that he is sent after Eve. The Director needed this job to be "off the books".

Revealing mistakes

When practicing she is wearing toe shoes, later seen walking to dressing room in slippers, then takes off toe shoes.

Miscellaneous

When Javier (David Castañeda) reveals his back tattoo it reads "vincit qui sSe vincit", which is grammatically incorrect, should be written with one "s" only (vincit qui se vincit). That is Latin for "he who conquers himself, conquers".
Eve is roughly 25 years old at the main time line point, yet Ana de Armas was in her mid 30s during filming.

Errors in geography

In the first fight scene the intruders her father shoots and (kills) disappear, and are not visible when fighting the second/third wave.
The trains which separate Eve from Pine when they fight the cultists on the riverside are green, but in Prague, where this is supposed to take place, no public transportation units use green livery. They use only shades of red and white (and for the suburban trains, blue and white).

The scene was actually shot in Budapest, Hungary, on the H7 suburban railway line.
The green train with the white stripe that almost runs over Eve outside the Prague Continental is an LEW MXA light train. This type was specifically manufactured for certain suburban lines in Budapest, Hungary. It was never in use in Prague.

Plot holes

Eve's father tries to sneak out of the house with her, with the house being somewhat dark it would have made sense for him to turn the weapon light on his handgun off. Walking around with the light on gives away his position.
Neither Ruska Roma nor John have any obligations to each other after the events of John Wick 3. Additionally, John Wick series director Chad Stahelski confirmed that the first three movies took place over roughly a week to a week and a half, and the time between Winston shooting John Wick (at the end of John Wick 3) and his recovery under the Bowery King was around eight months. Therefore, it would be impossible for the Director of Ruska Roma to send John Wick after Eve.
Twelve years after her enrollment into Ruska Roma, Eve is pictured practicing as a ballerina in front of the director. Furthermore, it is evident from Lena's final statements that she is 21 years old. Thus, Eve should have been 20 years old or younger and Lena 21. But the off-screen age of Lena and Eve does not correspond with this timeline.

Character error

While pointing to the map, Frank says the cult is located somewhere between Attersee and Feldkirch. However, on the map he's pointing to the Traunsee, with the Attersee visible to the left.
Eve is frequently shown holding and firing handguns "gangster" style, especially in the training montage. "Gangster" style is when a handgun is held and fired sideways, often by gangsters in movies, and wanna-be gangsters in real life because they saw it on TV and thinks it looks cool. In reality a professionally trained shooter would not hold a handgun in this manner, it not only has no practical advantage but has several disadvantages. Firing a handgun sideways makes the gun less accurate since sights were not designed to be aimed in this manner, and it makes controlling recoil more difficult, making quick follow up shots less accurate. Most importantly firing a handgun sideways, or any automatic firearm with a side ejection port, increases the odds of a stovepipe jam, where the spent shell casing doesn't eject properly and gets caught in the bolt/slide as it closes.
