Continuity

When Jonathan, Alex, his grandfather and Lista meet by the river, the moon is full, but it was only three-quarters full the night before. However, this could have been a creative choice by the filmmakers to show that "everything is illuminated" in that scene.
When the three run out of gas and camp outdoors, Jonathan briefly holds the photo of his grandfather and Augustine with both hands. Between shots, his hands are in different positions.
When Jonathan takes Augustine's pendant from the wall under his grandfather's portrait, he looks to the portrait and the bag is still there. Then in the next shot the bag is in his hands again.
In front of the train station, Jonathan explains to Alex that he is distressed by dogs as Alex gets into the vehicle. Jonathan miraculously ends up in the vehicle after both the driver and passenger are seated in a 2-door Trabant.
When the three meet the small kid who asks them for chewing gum, the kid's hands have different positions on the window when filmed from inside the car or from his back.

Factual errors

In the movie it is stated that the townspeople of Trachimbrod were murdered by Germans, while in fact the murderers were mostly Ukrainians with small Germans contingent. Source: sztetl.org.pl/en/article/zofiowka/5,history/
Francis Scott Key is credited for the "Star Spangled Banner". The composer, John Stafford Smith (1750-1836), should have been given credit rather than Key who was the lyricist. In the movie, the performance is played by a band and no lyrics are heard.
It would have been impossible to emigrate to America 'a week before (the Germans) came' to the village. An immigrant could not have traveled any direction except west at that time due to advancing German troops.
In the credits for the song "Start Wearing Purple," Eugene Hutz's band is credited as both "'Gogol Bordello'" (correct spelling) and "Gogol Bodello" (incorrect spelling, dropped "r").

Incorrectly regarded as goofs

When Alex's grandfather asks Alex and Jonathan to leave, Jonathan immediately leaves, even though Alex has not translated the request into English. This, however, was most likely a creative choice to show the power and emotion of Alex's grandfather's words.

Revealing mistakes

Miscellaneous

The train station purportedly in Odessa has very large lettering indicating that it is actually in Lviv.

Anachronisms

When Lista takes out a ring she has saved, it is from a modern baby food jar from the USA.

Audio/visual unsynchronised

When the Grandfather repeatedly sounds the horn of the car, he presses the middle of the steering wheel to do so. In the Trabant, the horn is activated by pushing the signal light lever forward.

Crew or equipment visible

In one scene when they are driving, you can see the car which is carrying the film/sound crew, over the grandfather's shoulder.
