When Mastroanni died in 1996, the Trevi Fountain in Rome, associated with his role in La dolce vita (1960), was symbolically turned off and draped in black as a tribute.

From 1970 to 1974 he had an intense relationship with french actress Catherine Deneuve. She was at his bedside when he died, along with their daughter, Chiara Mastroianni.

His three Oscar nominations for Divorce à l'italienne (1961), Une Journée particulière (1977), and Oci ciornie (1987) are the record for a performer in a foreign language film. The only other performers with multiple Oscar nominations for foreign language films are Sophia Loren, Liv Ullmann, Isabelle Adjani, Javier Bardem and Marion Cotillard with two each.

Since 1998, a "Marcello Mastroianni Award" is given to the best "first time" young actor/actress at the Venice Film Festival.

In 1976, estranged wife Flora Carabella reportedly offered to adopt Chiara Mastroianni because Deneuve's career kept her away so often.

Lived with Faye Dunaway from 1968 to 1970. Dunaway wanted Mastroianni to give her a child but only under the condition that he divorce Flora Carabella first. He refused, so she broke up with him. In her 1995 autobiography Looking for Gatsby, Dunaway wrote: "I wish to this day it had worked out." In the documentary Faye (2024), she described Mastroianni as the love of her life.

Along with Dean Stockwell and Jack Lemmon, he's one of a few actors who had won twice the Best Actor prize at the Cannes Film Festival.

Per a Mastroianni biographer, his mother revealed shortly before her deathbed that she was a Jewish immigrant from Belarus, a heritage she had previously hidden.

He is buried in the Cimitero Monumentale del Verano in Rome, Italy.

Two funeral services were held for Mastroianni. The first was at the Church of Saint-Sulpice in Paris on Dec. 20, 1996. His coffin was then transported to Rome where a second service was held Dec. 22 at the Verano cemetery.
