The layout of the windows in Dr. Melfi's inner room/office is illogical. That is, in an early episode, we see Tony dodge Silvio in the hallway of Melfi's office building. It appears to be a typical medical plaza building with multiple physician's offices on that hallway (e.g. the dentist Silvio was visiting, the office into which Tony ducked, and Dr. Melfi's office which we also see.) We see Melfi's office here in the middle of the hallway, not on the corner of the building. Therefore, the two sets of windows in Melfi's inner office are illogical. The windows in front of Tony (behind Melfi) make sense, as they are in a straight line from the hallway. However, the windows to Tony's right are not possible, as they would be looking into the office of the neighboring physician's office on the hallway.
In season 3 Paulie says Green Grove retirement home costs $8,000 a month, then in season 6, he says it costs $4,000. He asks Tony if his mother had the Silverbird package, which was the best package offered by the facility. This implies cheaper packages are available, which Paulie might have had to settle for due to not getting the full 50K he was expecting.

Factual errors

Several episodes show convicted felons (like Phil Leotardo) visiting prisoners who are incarcerated.

Unless they are blood relatives, this is not allowed by most prison systems, including the ones shown in the series.
Throughout the series the Soprano family very much follows Italian tradition. Yet when it comes to Anthony Jr. and Meadow's names they seem to deviate from tradition. Italian tradition dictates that the first born Italian son should be named after his grandfather. Hence, Anthony Jr. should have been named Giovanni or John after Tony's father. Likewise Meadow should have been named either after one of grandmothers; Livia or Mary, or at the very least have one of their names as her middle name.

Revealing mistakes

Meadow and AJ attend Verbum Dei School, which is a Catholic school. Yet neither of them, or other students, are seen wearing uniforms which are required in these private schools.
In many instances, characters are seen firing semi-automatic pistols (most commonly Beretta 92s). These weapons are clearly stage guns, as the pistols are seen firing multiple rounds with no movement of the weapons' slides.
None of the characters based in or from New York speak with a New York accent.

Angelo Massagli should be credited as Bobby Baccalieri III, not Bobby Baccalieri Jr.

Character error

On several occasions Dr. Melfi looks at her watch during her sessions with Tony which is considered unprofessional. She could have set a timer or an alarm as most psychiatrists do.
Tony Soprano takes the medication Prozac as a part of his treatment but is seen throughout the series consuming alcohol. Consuming alcohol and the medication within a short time period of each other is considered dangerous by psychiatrists and wouldn't have any effect.
Carmela occasionally refers to "Neil Mink" as "Neil Mig"
