That scene broke my heart :(( She was just a poor lady that wanted to be loved. I think the red dress was a very good representation of the wishes of Sara, she wanted to fit into the dress again being the same as she wanted to return to the past when her life was "perfect", or at least when she was a better version of herself and she was loved. Also showing how an eating disorder and lack of self-love can be just as lethal as drug abuse, and even worse when she combines them. I think the star of the movie was Sara, just as she wanted to shine on television.
