Dr. Kenzo Tenma disobeys orders to operate on a wealthy donor, following his conscience to operate on a small boy named Johan instead.

Dr. Tenma saves Johan's life, but Mayor Roedecker dies during surgery under Dr. Boyer, prompting Dr. Oppenheim to send a report to the director.

Inspector Lunge suspects Dr. Tenma in the deaths of three doctors and the disappearance of the Liebert twins; however, he is eventually promoted to chief of surgery. Nine years later, he meets a criminal who warns of an impending monster.

Junkers is interrogated about the murders by Inspector Lunge, and Dr. Tenma persuades him to confess. Junkers flees after discovering his guard has been killed, and Dr. Tenma pursues him.

While Dr. Tenma investigates the serial murders of middle-aged, childless couples and Johan's true identity, law student Nina Fortner receives poetic emails from a stranger.

With the help of Maurer, a local newspaper journalist, Dr. Tenma is able to track down the Fortners on the day of the twins' twentieth birthday.

Dr. Tenma and Nina arrive at a gruesome scene, finding the Fortners and a journalist dead. Anna's confession of shooting Johan brings back memories.

Dr. Tenma returns to work at a hospital in Dusseldorf, but is forced to flee when the police suspect him of killing an elderly couple in Verden. Meanwhile, Eva pleads with him to take her back.

Dr. Tenma undergoes intense firearms training with a former army veteran named Hugo Bernhardt, who takes care of an orphaned girl after shooting her mother in Myanmar.

While investigating the murder of a prominent councilman and his wife at the Springer Estate in Verden, Tenma encounters the petty criminal Otto Heckel and escapes with him.

Tenma learns that Johan used to be in an orphanage known as Kinderheim 511, where he meets an injured boy named Dieter and a former East German official named Hartmann who used to work there.

Dieter is picked up by Hartmann before Tenma could return, and Tenma learns more about the experiments in Kinderheim 511.

In a small village, Dr. Schumann worries over the health of his sick but stubborn old friend, Petra.

Workaholic Inspector Lunge is so focused on his job that he neglects his personal life. When a key suspect in his case commits suicide, he becomes obsessed with catching Tenma. Meanwhile, Eva Heinemann is dissatisfied with her life.

Tenma investigates the Heidelberg murders and learns of a right-wing nationalist organization that's planning to burn down the Turkish portion of Frankfurt. Nina is captured by the organization.

Nina finds Ayse dead at Baby's mansion, where Goedelitz and his men have been killed. Tenma is captured by General Wolf's men, but he sends Dieter to warn the Turkish community.

Tenma saves the Turkish community from a neo-Nazi fire plot, while Nina learns that Johan has a split personality.

Nina recalls her time working at an Italian restaurant and taking marksmanship lessons. She learns that her boss, Mr. Rosso, is a former assassin. She leaves to continue her search for Johan.

Tenma visits Dr. Gillen, a former rival, for help in understanding Johan. Dr. Gillen initially turns Tenma over to the authorities, but changes his mind after consulting with a prisoner who provides evidence of Johan's existence.

Tenma and Dieter hitch a ride with a former police officer and his wife.

Former detective Muller is lured into a trap by Johan, who has his bodyguard Roberto kill a private investigator and kidnap Nina.

Lunge sets a trap for Tenma, but is wounded by the real criminal.

Eva Heinemann is arrested for public drunkenness and is also homeless after not paying her rent.

Eva trades Johan's photos for Tenma's location.

In Munich, Karl Neumann and fellow student Lotte Frank engage in a series of events involving the elderly, blind, and wealthy Hans Georg Schuwald.
