The official cause of death was toxicity from fentanyl and para-fluorofentanyl, a synthetic opioid more potent than fentanyl, according to the agency, and her manner of death was listed as accidental.

Autumn Crittendon (aka Autumn Oxley), who appeared on the fifth season of MTV’s 16 and Pregnant reality show, died Saturday at a home in Henrico, Va, police confirmed to Deadline. She was 27.

Police and paramedics were called to the home at around 3:12 pm Saturday on a report of a medical emergency, according to a statement from the Henrico Police Department provided to Deadline. Crittendon was found unresponsive and attempts by paramedics to revive her were not successful. She was pronounced dead at the scene.

Henrico County Pd have classified the incident as a death investigation and are working with the medical examiner to determine the exact cause and manner of death.

16 and Pregnant followed Crittendon’s pregnancy and birth of her eldest son Drake in the show. She went on to have two more children.
