When Judah the pastor hands over the book at church he is suddenly wearing a white blouse.
At around 47 minutes, when Julie and Ava are talking at the table, Julie's hand switches between holding the coffee cup and not holding it.

Incorrectly regarded as goofs

When Teddy "causes" the crash that starts the whole thing, he was walking back to the car, technically just crossing the road. The driver clearly wasn't watching the road properly and was going too fast then they all tried to save him and called the police and ambulance. The killer coming after them for it doesn't make any sense as a "revenge"
Tyler's death likely wouldn't have been labeled a suicide, at least not so quickly like it is said in the story considering the wound in her neck.
The airbags should have deployed in Sam's car when it hit the barrier prior to falling over the cliff.

Revealing mistakes

Danica's text to Ava in the church noticeably sends itself before Madleyn Cline hits send, revealing that, as is commonplace in movies, the contents of the phone screen were added in post production.
As is common with many films, the photo that shows Julie, Barry, and Helen as teenagers is clearly an on-set photo from the first movie. All of the actors are in the costumes they wear in the film.

Miscellaneous

When the giant round cage chandelier falls on Ava, imprisoning her, why doesn't she just knock it over and crawl out the bottom instead of trying to open the door? If it's light enough to be suspended, it should be light enough to be pushed over.
After Milo and Ava make out, he goes downstairs to check his phone. It says 10:45 p.m. on it. After he's been killed, Ava, who's been waiting upstairs the whole time, text messages him. Her phone shows it to be 11:55 p.m. It is highly implausible that she would have waited an hour and ten minutes without checking up on him sooner.

Errors in geography

During Ava's first meeting with Julie, she says South Port is 45 minutes away from where Julie lives. However, at the end when Ava tells Julie that the killer was Stevie, it takes Julie maybe 5 minutes to show up at Ray's bar in South Port.

Plot holes

At the police station Danica wants to go back home ASAP. A) Her home is an active crime scene and therefore any non-official could enter the premises B) Her fiance was just slaughtered hours ago at their home with blood all over the place, going home to such conditions so soon does not feel plausible.
After surviving the attack Danica seems very easy going and relaxed about cuddling with another man in her wedding bed very soon after her fiance was brutally murdered.

Character error

The main characters are divided regarding whether or not Teddy caused the accident, with some claiming that it happened because he was "standing in the middle of the road." In fact, ALL of them are in the road. There is no breakdown lane where they are; they parked their car illegally on the road and barely even pulled to the guardrail. Any car in the right-hand lane would need to swerve to avoid all of them, regardless of Teddy's whereabouts.
When Ava is at Julie's house she looks at a photo of Julie, Helen, and Barry from the first film. Julie then comes into the room and accuses Ava of "going through (her) stuff." Ava was looking at a framed photo on Julie's mantle; she is hardly going through Julie's belongings.
