When Harley emerges from the limo to meet the president, her "Daddy's Lil Monster" tattoo on her left shoulder is heavily concealed with white makeup. However, in the next scene, which follows immediately afterwards, the tattoo is clearly visible.
At the beginning of the film behind the Suicide Squad is a large American flag painted on the wall by itself. However in the next cut as the Squad is being introduced; there are now several soldiers standing in front of the flag. In addition, when Javelin is introduced he now has a large Jeep behind him instead of the flag.
At the end of the final fight between Flagg and Peacemaker, Flagg is stabbed in the center of his chest by a tile shard through the heart. when he rolls over after delivering his final line there is no puncture in his shirt nor shard visible in his chest.

Factual errors

Around 59:09, when the Squad arrives to "La gatita" club, there is a sign in the wall with a misspelling word in Spanish. It reads "No se permite alcohol más aya de este punto" (Alcohol forbidden beyond this point). The word "aya" should be spelled "allá".
The mission is black ops, yet Captain Flag wears a uniform with an American flag on it. This violates military protocol as you don't want to be identified when carrying out black ops.
When the team is first shown flying in the V-22 aircraft an insert shot shows the flight deck and the instruments. There are four sets of engine instruments but the V-22 is just a two engine aircraft.
There is no gun able to give a bullet enough kinetic energy to go through a bigger bullet (even if both bullets were shaped exactly like the ones used by Bloodsport and Peacemaker), let alone doing that and also going through the other shooter's gun and hitting them.
At 1:26 The Thinker says those morons set the explosives off too early, resulting in the subsequent collapse of the floor above. As RatCatcher2, Peacemaker, and Flag recover from the collapse none are covered with any debris,

Incorrectly regarded as goofs

Severed heads are not able to move eyes or make expressions, unlike the one that is shown up close between King Shark's teeth after he beheads a soldier.

Actually, they can. There are many reports of heads reacting to outside stimuli for up to 30 seconds after decapitation. Charlotte Corday, assassin of Jean-Paul Marat, was a famous case. Henri Languille's post chop facial contractions were reported by a Dr Beaurieux in 1905.
Blackguard was under heavy surveillance, first in the prison, and afterwards among his Suicide Squad teammates. So he wouldn't have had a chance to sell the Squad out. But his team's attack was always a distraction tactic - Walker needed the army to be there to meet it so will have had a hand in making it possible.
The shark receives hundreds of shots, all the time, but no bullet pierces his skin, so it would be impossible to the mission to implant a bomb into his head.

The bomb could have been implanted with a specialized device to cut open King Shark's skin or have been inserted through another opening like his eyes, nose, or ears.
Sebastian the rat is shown throughout squeaking a lot. Rats only squeak when there's danger coming, they're in danger, or when they're being injured.

Sebastian the Rat is clearly hyper-intelligent. He is squeaking so that the humans can hear him.
The shark receives hundreds of shots, all the time, not even a single scratch, then a small piranha fish bites it and He is bleeding to death. Guess army should just start using piranha teeth as bullets since it is harder than steel right?

The "piranha fish" are aliens with unknown jaw structures and compositions. And King Shark was never "bleeding to death." He gets up and continues fighting like normal immediately after the aliens stop biting him.

Audio/visual unsynchronised

When Task Force X/The Suicide Squad are sat round the table drinking Colonel Flag can be seen rubbing Polka Dot Man's head and saying "There you go. There you go." but his mouth is not moving when he says this line (@ around 1 hour 1 minute).

Crew or equipment visible

At 31:53 three members of crew are visible as the squad walk through the jungle.

Plot holes

After the main tower explosion the big starfish breaks the remaining walls in the tower. Even when the whole thing collapses over it, the star just stand up as it the concrete weighs nothing. So it could break the walls since the beginning.
The Squad is the number one enemy of the Colto Maltesian army, but when the heroes run out of the collapsing Jotunheim, into the arms of the army, nobody shoots at them. This is strange, as Suarez doesn't know Starro is free, so his commands aren't changed yet.

Character error

At 13:38, Weasel's name is clearly misspelled as "WEASAL" on one of the ARGUS operation screens despite correctly being spelled in every previous instance.
Bloodsport mocks Peacemaker for wearing a helmet he calls a "toilet seat". Considering the shape, he likely meant a "toilet bowl".
