Yes. Originally the creature was going to be a 3D animation, however the person assigned to create the creature backed out of the project (after being paid). So the director went old school.
The van was a remnant of an earlier script that was part of a TV series. Each episode was to have this van which, in the last of three episodes, would tie all the stories together. It was to end up being a van from a mental hospital and the main character was to be carted away. It was left in for "intrigue" and for a tie to a future movie(s).

It represents the fractal nature of the universe. As you zoom into a fractal you see that the fractal contains a form of itself. It also represents universes within unverses on the theory that blackholes contain other universes which in turn contain blackholes, and the concepts of "as above, so below" and "as in the macrocosm, so in the microcosm". And since this sequence is a dream, it represents a dream within a dream within a dream...etc. Thus is the dilemma of the main character in the movie. It is also reminiscent of neural nets. Each node in the fractal is another universe or a dream.
