At the end of John Wick: Chapter 3 - Parabellum (2019) is shot by Winston and sent tumbling off a roof. At the start of this film, John is seen training, with his wounds fully healed. After everything John went through, he likely needed an extended period of recovery, as well as additional time to train and get back in to shape. So the film likely takes 6 months to a year after chapter 3.
