When "Claire" meets her birth mother, "Meredith", she tells her that her adopted family named her "Claire". In this episode Eric Roberts character mutters "Claire's" name and "Meredith" recognizes it. Based on the initial meeting of mother and daughter, "Claire" must have had a different name which was never revealed.
