Factual errors

At three minutes Wanda throws the car into Agnes and crashes it into the house. She walks up to the wreckage and looks into the front window. In the glass she sees a reflection of the white Vision descending behind her. But she is standing at an angle to the glass, so the actual Vision reflected in the glass should be approaching from Wanda's left, and she should turn to her left to see him. But she looks over her right shoulder and sees him directly behind her. That is not how actual reflections work.

Incorrectly regarded as goofs

The identity of Jimmy's witness, whose welfare he arrived in Westview to check on, is not overtly referenced, but a close examination of the documents this man owned reveals him to be "Ralph Bohner" (an alias).

Revealing mistakes

When Agent Woo removes his handcuffs he has both sides unlocked when he brings his hands around in front of him. It would have been more efficient to either leave one side locked, or unlock the second side in front of himself.

Plot holes

The real Vision's directive is just to kill the fake Vision, yet for some reason he tries to harm Wanda as well.
There was no reason to send the government-created version of Vision into Westview since Hayward would have known that the version created by Wanda would disappear once the Westview reality collapsed.
