The original screenplay was written by Zach Chassler, who had worked with director, Jeremy Kasten on three other projects previously: The Theatre Bizarre (2011), The Wizard of Gore (2007) and The Thirst (2006).
Muse Watson was cast because director Jeremy Kasten and casting agent Pat Moran had both been captivated by his role in I Know What You Did Last Summer (1997).
For one scene, a professional maggot wrangler was brought in to cover actress Katie Foster in live maggots.
The four actors who play the lead roles, were chosen from hundreds of local Baltimore actors by Jeremy Kasten and local casting agent Pat Moran, who had worked with Kasten previously on various projects.
The school was used as an on-location set, as well as make-up, wardrobe, special effects, actors and extras housing, and an entire green screen soundstage. Because the building was largely abandoned, each department had their own space.
