After five years, While Ömer was trying to find a place for himself in the surgery department of Private Gümüsok Hospital, he lost his patient in an operation he underwent. Ömer finds himself in Private Uluçinar Hospital, a town hospital far from Istanbul. Trying to get rid of this hospital as soon as possible, Ömer's path will cross with Hakan Hoca.

Hakan immediately takes Leyla to surgery. While Ömer tries to prevent the woman he loves from becoming disabled, he witnesses how skilled a surgeon Hakan is.

Erol Gümüsok wants Hakan to perform the surgery. When Hakan objects, he threatens him using a secret from his past. Meanwhile, a friend from the past will extend a helping hand to Hakan, and make Hakan put a trump card on the table against Erol. Ömer is determined to give up his resignation and leave because of Hakan, whom he cannot agree with. It seems that the efforts of Ihsan, Mine and Turgut to dissuade Ömer will not work. While the wounds on Leyla's wrist heal rapidly, the wound in her soul becomes heavier. Although Leyla wants to stay in the hospital and work with Hakan, Hakan has a nasty surprise for her. Yalçin, on the other hand, is furious that Erol's surgery will not be performed in his own hospital and mobilizes all his means to turn this situation in his favour.

While Hakan is dealing with the hospital, he is also making plans for Erol Gümüsok's surgery. Ömer is in for a shock when he learns that Kemal Demir, whom he has been looking for for years, is Hakan Hodja, whose star he has never reconciled with. Knowing very well that in order to be a good surgeon, he must grow up with a good teacher, Ömer rejects the offer from Private Gümüsok Hospital. While everyone thinks that Leyla will not accept to be a maid and will leave the hospital, Leyla starts working as a janitor. A big surprise awaits Ömer in an operation he has undergone. The nasty surprises waiting for Ömer will not only be limited to his job, but will also put an unexpected obstacle in front of his love for Leyla. Despite all of Erol's warnings, Yalçin has no intention of leaving Hakan alone. On the other hand, Mine, who went out to look for Hakan, sees him with Arzu, and his bad memories from the past come to life and the duo comes to a standstill.

Yalçin's sending a team to Uluçinar, using Erol's surgery as an excuse, upsets everyone, especially Mine. Realizing that he is drawn into an inevitable war, Hakan decides to play the game according to the rules. When Ömer shares the news about his mother with Leyla, the ice between them begins to melt. Bülent, who found an informant in the hospital, implements his plan to expand his team and isolate Hakan. Bahar Nurse from Bülent's team goes over Leyla's trauma. Berk, on the other hand, uses Sevil, who is grateful to him for saving his father, to get information about Leyla. Mine secretly meets with Yalçin, who wants to meet with him to protect Hakan; but he is not aware that he is being watched by an enemy. Yalçin gathers trump cards to attract everyone, especially Leyla, to his side. While all this is going on, a suspicious person walking around the hospital all day will endanger everyone's life.

When the patient, who is understood to be a rapist during the operation, which continues under great stress, becomes paralyzed, question marks arise about Hakan's wrong operation. When Berk tells Yalçin a detail about the surgery, Yalçin gets a big trump card. Yalçin sends an inspector to Uluçinar to investigate Leyla and Hakan. Ömer makes a move to save Leyla; but this turns all of Hakan's plans upside down. Hakan is dismissed from the profession. Leyla cannot withstand the psychological test and is forced to resign.

Despite all the pressures of the inspector, Hakan, who goes into Ihsan's surgery, is banned from the profession. While returning to Uluçinar Hospital, Leyla and Ömer witness a chain traffic accident. Since there is no doctor other than Hakan in the hospital, Hakan continues to intervene with the injured, despite being banned from the profession. This situation frightens the whole team, especially Mine; But Hakan has another plan. A little girl injured in an accident changes the course of events. Since Yalçin could not get the performance he expected from Bülent and Berk, he starts to think about other formulas. Ömer reaches his mother as a result of long efforts, but a big surprise awaits him.

Erol's falling ill and coming to the hospital upsets all the balances. Hakan has to quickly assemble a team to take the surgery forward. Yalçin does his best to isolate Hakan. Yalçin tries to lure Ömer to his side by claiming his mother's treatment. When Leyla learns about Ömer's lie about his mother, they fall apart. Thanks to a plan, Hakan shoots Yalçin with his own gun and melts the ice between him and Mine. Things change with the arrival of Ömer's mother to Uluçinar. Omer, who is forced to do something he does not want by Yalçin, is cornered when he is caught by Hakan.

Omer, cornered by Yalçin, has to choose between his mother's treatment and his conscience. Hakan learns that Mediha's doctor is Yalçin and decides not to leave the square to Yalçin. Hakan and Mine quarrel over a photo that will shed light on their past. A patient relative from the past puts Ömer in a difficult situation and affects his decision about the report. Ömer's decision moves the stones. When Erol's condition gets serious, Hakan has to decide on a more difficult operation. The assistant race between Leyla, Ömer and Berk heats up. With only a few days left until the surgery, a great danger is at the door that will affect everything.

While Hakan and his team continue to work on Erol's surgery, the Mers panic in the ED turns all plans upside down. While Hakan and Leyla stay outside, Mine and Ömer have to go into quarantine. Leyla risks her chance to be an assistant in Erol's surgery and enters the ER to treat Ömer. Meanwhile, Yalçin and his team are not sitting idle. Yalçin puts his plan into action, which will put everyone, including Erol, in great trouble.

When the test results come, it turns out that the disease that caused the quarantine of the emergency room is not MERS. While the whole team is breathing a sigh of relief, another problem arises, Mr. Erol has advanced lung cancer. This situation puts Arzu, who is collaborating with Yalçin secretly from Hakan, in a difficult situation. Mr. Erol wants to have the surgery, but Hakan is torn between his ethical values and the demand of his patient. Uluçinar has to make a difficult decision for his future. As Leyla learns the secrets about Mediha's treatment, she begins to be overwhelmed by what she knows. This causes her to be cold to Ömer, while Ayperi's attack makes him jealous. Hakan disappears for a while before making his decision. Meanwhile, Ömer has to take all the responsibility. Will Hakan return?

Towards the day of the surgery, the pressure on Hakan, Ömer, Leyla and Mine increases. Since Leyla cannot tell Ömer the secret she learned about Mediha, she starts to stay away from him. Despite all of Mine's concerns about the surgery, the support she gives to Hakan strengthens the bond between them. Meanwhile, Arzu and Yalçin make a plan. Yalçin forces Hakan to make a choice. Hakan will either switch teams to operate with the surgeons of Gümüsok Hospital or perform live surgery under pressure in front of a crowded team of surgeons. Hakan does not leave his team and accepts all the difficult conditions and goes into surgery.
