Jon Taffer has said in interviews that they tell the bar owner they are one of three bars in the area that might be selected. They install the hidden cameras and have a camera crew on-site for a few days. In reality they really one have selected one bar but they want the owners to be caught off guard and surprised when Jon Taffer walks in.
In interviews v said their budget is big, in the $100,000 per episode. But many of things they install like a POS system, bar taps, seats, tables are on lease, the show covers the fee for a few months then it is up to the owners to take over the lease terms.
In an interview Jon Taffer said the "Turtle Bay" bar in New Orleans has been the most successful bar rescue. He said the barber chair (shot chair) he installed has brought in up to $25K per month in sales alone.
There have been 7 seasons and over 100 episodes. Success can be judged different ways. Overall doing some basic research it isn't so great.
Many bars have gone out of business after the show. But most of the time it is less of what Taffer did and more about ownership and management post-show. Bars can be renovated to look nice but a bad manager is still a bad manager.
