After watching Tom Six's first two Human Centipede movies, accountant Dwight Butler (Laurence R. Harvey) comes up with a plan that might help psychotic warden Bill Boss (Dieter Laser) cut down the costs of running his prison and deter anyone else considering a life of crime-connect all the prisoners to create a 500-person human centipede.

Human Centipede 3 is the third movie in Dutch film-maker Tom Six's Human Centipede series, preceded by The Human Centipede (First Sequence) (2009) and The Human Centipede 2 (Full Sequence) (2011). Six wrote the screenplays for all three movies.

Boss and Dwight present their human centipede to Governor Hughes (Eric Roberts) who walks out in disgust. However, as he is leaving, the governor reconsiders and returns to tell them that a human centipede is "exactly what America needs" and plans to use his support of the human centipede to further his political career (perhaps even a presidential nomination). Boss then shoots Dwight for taking credit for the centipede (despite it being his idea). The film ends with Boss watching over his centipede from his watch tower in delight as the Star Spangled Banner plays and the credits roll.

Dr. Heiter wakes up and it is revealed that all 3 films were a dream that he had. At this point he has already created a centipede out of his dogs (and failed) however the dream has now given him the idea to create a centipede out of people. He then goes out to find 3 victims which restarts the chain of events.
