While Brian has his first day on the rig, Johnny runs into Theresa for the first time since they took "a break," and Hank and the guys discuss their "In Case of Emergency" contacts.

A lightning strike at a church picnic gets the guys to start thinking about how there's no justice in sickness and injury, because like it or not, the "rain falls equally on the just and the unjust." After a discussion of karma, the guys volunteer to teach CPR to disadvantaged kids only to later be given killer Bears tickets for the same day they're supposed to teach.

A patient pleads with the guys to back to his house and "clear history" on his computer. After stumbling across a troubling image on the computer the guys can't get it out of their heads.

After losing a man that reminds Johnny of his estranged father, Johnny decides to reconnect. Meanwhile, Brian is touched by the dying man's last words and is determined to delivery them in person.

After a drunken booty call at Johnny's place, Theresa suggests that they start a regular, no strings attached, intimate relationship. His colleagues encourage him to enjoy it.

After it's revealed that Hank is deathly afraid of spiders and Brian comes face-to-face with his suicidal doppleganger, everyone examines their own fears, and Johnny gets payback for all the practical jokes he's played.

While teaching a class to prospective EMTs, the crew recalls a day that ran the gamut from the mundane to the catastrophic, and Johnny tries to find out who ate last the donut.

After a close call on the job, Theresa decides to move to Washington and work for the FBI. The only way Johnny thinks he can convince her to stay in Chicago is by proposing to her.
