The year is 2016 and it's the night of the Supermoon. Skylar is on her first day of residency as a surgeon at Republic Hospital. But her nervous excitement gets thrown out when she ends up holding the hands of a dying patient.

Despite turning down a marriage proposal, Skylar is in high spirits. There's a spring to her steps and a smile that reaches her eyes. Something that's been missing for a long time. Skylar heads back to the hospital.

Skylar and Ryan hit it off again and vibe well with each other. Ryan teases her and gets her off her feet and into a dance. Skylar laughs, forgets her world and swirls to the music that only she can hear. Derek gets a second chance.

Skylar is at the hospital's resident counselor's office. She is freaking out if she's crazy. Did she make up Ryan in her mind? She's asked to take a leave of absence from her surgical duties. Skylar resists the order but has no choice.

Skylar has made peace with the idea that she went crazy temporarily. She tries to get on board with all the textbook methods of finding sanity. Maybe Jeffrey was her anchor after all.

Derek is Ryan's elder brother. Ryan is a budding artist who paints surreal pieces and is constantly on the lookout for muses. Ryan spots Skylar in the middle of a busy event and is instantly enraptured by her unconventional looks.

Skylar remains frozen as she hears the story of how Ryan was killed. She now knows that Derek and Ryan are brothers. And that she's not crazy. Well, not completely. But at least she didn't dream of Ryan. He really did exist.

Ryan appeals to Skylar to tend to Derek's heart and to bring him out of his self-imposed darkness. He reveals that Skylar is his muse and he can't make contact with anyone else. The only who can see him is Skylar.

Derek confronts Skylar about the coincidences that don't add up. Skylar, under much duress, admits that she can 'see' Ryan. Derek freaks out and lashes out at her, connecting her to all the misery and mystery.

Everything's a complete mess and Skylar needs to fix it. But she needs to admit the truth first. When she tells Ryan about her oversight, Ryan smiles. He has always known the real reason. He feels that only she can heal his brother.
