🔔 Why is Eli Lilly moving deeper into infectious diseases? With the recent vaccine acquisitions, Eli Lilly is deliberately building a third franchise pillar. Infectious disease now sits alongside metabolic disease and oncology as a strategic priority, not an adjacency. Vaccine expert at Airfinity, Patricia Delgado says: “The core thesis is interception. 🦠 Case counts and deaths continue to rise. With no approved vaccines or therapeutics for BDBV, there is an urgent search to identify what can be deployed. Airfinity’s evidence assessments have identified the countermeasures most likely to be used as the response scales. 🔬 Therapeutics landscape: remdesivir and MBP134 as leading options → Remdesivir: near-certain for deployment. An existing key feature of therapeutic options deployed in Ebola outbreaks, with local manufacturing available in Egypt; no BDBV efficacy but retains potential utility as a broad-spectrum antiviral. → MBP134 (Mapp Biopharmaceutical): monoclonal antibody candidate with the strongest BDBV-specific data, showing 100% survival in a lethal-dose non-human primate challenge. Both therapies are now in WHO’s Phase III adaptive filovirus trial, PARTNERS, creating a formal route for rapid deployment. With Africa CDC considering remdesivir and MBP134, near-term field use appears likely, potentially as co-deployed antiviral–monoclonal therapy to maximise efficacy. Additional potential: → Obeldesivir: an orally bioavailable prodrug of remdesivir also under consideration by the Africa CDC. As the two are mechanistically equivalent and neither has NHP data for BDBV, obeldesivir would likely replace rather than complement remdesivir if selected. 💉 Vaccines landscape: Ervebo as likely default, with a potential wildcard → Ervebo (rVSV-ZEBOV, Merck): held in the global Gavi stockpile. However, it targets Zaire ebolavirus rather than BDBV, leaving cross-protection uncertain and limiting ring vaccination utility. → Potential University of Oxford candidate: under discussions to develop, but must complete trialling before deployment is possible. Serum Institute has indicated it could manufacture doses within 30 days if required but regulatory pathways would be a key constraint. As the Ebola situation evolves, so does our intelligence. 🦠 The DRC has declared its 17th Ebola outbreak, and this is only the 3rd outbreak in history caused by Bundibugyo ebolavirus (BDBV). On 17 May, the WHO issued a PHEIC, the first ever without prior Emergency Committee recommendation. This is also the first #PHEIC involving #BDBV, a strain for which no approved vaccines or therapeutics exist. 🚢 A hantavirus outbreak aboard the MV Hondius has crossed vessel boundaries, and the strain at its centre is what makes this situation worth watching closely. 🦠 On May 6th, the #WHO confirmed it was the Andes strain, the only #hantavirus variant capable of person-to-person transmission. Most hantaviruses don't spread between people but this one can. With an incubation window of 1-8 weeks and passengers from across 28 countries, new cases could still emerge through late June with passengers who have disembarked. This complicates contact tracing and clinical preparedness far beyond the ship itself. 📍 Airfinity's analysis points to Argentina's 2025/26 highest incidence hantavirus season as a likely contributing factor: 101 cases this season versus 57 last year, concentrated across Buenos Aires and Salta. Given the ship's departure during an active season, sequencing will be essential to clarify the relative contributions of environmental and secondary transmission. ⚠️ The World Health Organization and theECDC’s current low wider-risk assessment, pre-dates the Andes strain confirmation.
