On paper, I feel like some type of sampling-based code review would go a long ways. For instance, once a week each engineer reviews a few PRs that were merged that week. Once a month, everyone reviews a module/service/etc to assess code quality.
In practice, the companies I've worked with have relied heavily on gating what code gets merged to get people to do reviews. So how do you get people to review when you take away the "gotta review to merge" incentive?
I'd love to hear from anyone who has worked on teams that had effective code/system/architecture reviews that were performed _after_ code was merged. In theory this always would have been useful, review a completed project holistically and you catch things that fragmented PR review misses.
