If “Raised on AIRSWEB” was selected under “Action taken”, then observer can’t raise the report
without adding the AIRSWEB number.
“Applicable GSR” is to be changed to “LSR Violation”.

7 The line between Observer-1 & Observer-2 is to be removed
8 Leadership field to be mandatory … this is needed for easy extraction on excel sheet
