This waiver allows a child named ____________________________ to attend the Fellowship Baptist Church of Talisay's 2018 Church Youth Campference from November 1-3. The parent agrees to the rules set by the Campference Director and Church Workers and believes the camp will help their child grow holistically. Both the child and parent submit to the camp's policies and give authority to the Campference Director and Church Workers to responsibly carry out any actions related to the child's performance.

I am fully aware of and I agree with the rules set by those authority, particularly, the Campference
Director and Church Workers. I believe that this camp will help my son/daughter grow hostically and
will enable him/her to become a better person that God wants him/her to be.

I and my son/daughter fully submit to the camp policies, and give authority to the Campference
Director and Church Workers in responsibly and lovingly carrying out the corresponding actions based
on my child’s performance.
