AFFIDAVIT OF CONSENT TO ADOPTION

I, JAY P. GABRIEL, of legal age, Filipino, married, a greencard holder and


presently residing at 50 VAN BUREN AVE. ST. HOPKINS, MINNEAPOLIS,
55343, USA and domiciled at San Agustin, Isabela, Philippines, after having
been duly sworn in accordance with law, hereby depose and state:

1. That I am one of the children of Spouses BONIFACIO


VALDEZ-GABRIEL & ANANIA GALLARDO PACIS who
married on April 10, 1984 at Maddela, Quirino;

2. That I, as the second child was born on December 13, 1989 ;

3. That my parents are loving and caring that made me and my


sister made well in our studies;

4. That my parents have been a good provider to us, until I was


able to find a stable job as restaurant server at Loews Hotel
and Resort Minneapolis, USA;

5. That in 2006, or barely when I was only 17 years old, a female


toddler was turned over to my parents so they can provide
for her since her parents abandoned her;
6. That my parents named this child, JHO-ANNE PACIS
GABRIEL who became a sister to me and treated her as part
of the family;

7. I have provided for her as well together with my parents since


I started to work in the USA since December of 2013;

8. Believing that it would be for her own interest and benefit, I


hereby signify my written intent and consent, freely and
voluntarily to have my sister, JHO-ANNE PACIS GABRIEL,
be legally adopted, in accordance with law;

9. That I fully understand that as I consent to the adoption, she


will become part of our family legally and thus, with the same
rights and privileges of a biological child;

10. That because of our love and affection, I freely consent that
she may also exercise whatever rights she may have as the
true and legally adopted child of my parents;

11. That I execute this affidavit to declare my consent to the


adoption of JHO-ANNE PACIS GABRIEL, as well as to the
truth of the foregoing facts.
