KNOWN ALL MEN BY THESE PRESENTS:

I, ________________, Filipino Citizen of legal age, single with residence and postal address at
______________________________________, to be my true and lawful Attorney-In-Fact for me, in my
name, place and stead, to do all of the following acts, to wit.

To apply for membership, attend seminar, file, sign and execute housing loan application and mortgage
contract in connection with my application from the HOME DEVELOPMENT FUND for a loan in any
amounts that may be approved by the said institution, offering as security for the payment of said

To sign, execute and deliver promissory notes in favour of the HOME DEVELOPMENT MUTUAL FUND, as

To receive the proceeds of the promissory note which may said Attorney-In-Fact may execute in
connection with the mortgage contract relative thereto;

To encash, endorse, deposit or negotiate the check that may be issued by the Home Development
mutual fund in connection with said loan.
To transact and negotiate with the Home Development mutual fund and to the effect that the said
Attoryney-in-Fact shall be designated and named as the payee of any and all checks that may be due to
us from the Home Development Mutual Fund and.

To sign all pertinent documents regarding my membership with Pag-Ibig Fund.

Hereby granting and giving unto said Attorney-in-Fact full necessary power and authority to do
and perform any and every act requisite and necessary it be done in and about the premises as fully to
all intents and purposes as I might or could, if personally present and acting in person. Hereby ratifying
and Conforming all that said Attorney-In-Fact may also do or cause to be done under and virtues of
these presents.
