I, MAXIMO ACEBIDO, of legal age, Filipinos, with residence at Brgy. San Salvador,
Prosperidad, Agusan del Sur, after being sworn in accordance with law, depose and say
that:
1) I am the heir of my father SATURNINO ACEBIDO, owner of a real property under tax
declaration No. 621, with an area of three (3) hectares more or less, located in San
Salvador, Prosperidad Agusan del Sur;
2) That I am the actual occupant of such parcel of land since the death of our father;
3) That MERIAN FERNANDEZ is an applicant for Commercial Sand And Gravel (CSAG)
permit before the PMRB, PENRO-LGU and MGB of Agusan del SUr.
4) That the area applied for by MERIAN FERNANDEZ is adjacent to the real property in
my actual possession and covered by Tax Declaration No. 621.
5) I am interposing No objection and gave my consent to MERIAN FERNANDEZ being
the applicant for CSAG permit for the area adjacent to my land covered by Tax
Declaration No. 621.
6) This affidavit is being executed to attest to the veracity of the foregoing facts, to
support the application of MERIAN FERNANDEZ for CSAG permit, and for whatever
legal purpose it may serve.

C. OTERO (deceased), after being sworn in accordance with law, depose and say that:
1) I was informed that the Prosperidad E-Bamboo Association is a beneficiary of
"ENGINEERED BAMBOO PROJECT" by the Department of Trade and Industry, for the
livelihood of Senior High School students and Alumni association of Monato
Institute of Technology;
2) The project and its equipments will be set up in the premises of Monato Institute of
Technology, adjacent to the land and building owned by RUFINO C. OTERO;
3) That I, being the son of RUFINO C. OTERO-the neighboring land owner, will NOT
oppose/object on the said project to be implemented and the set up of the
equipments in the premises of Monato Institute of Technology
4) This affidavit is being executed to attest to the veracity of the foregoing facts, to
support the application for building permit of Marceliano Felix Monato/Monato
Institute of Technology, and for whatever legal purpose it may serve.
