Bevel washers should be used with anchor bolts that are not perpendicular to the surface being bolted. A pump foundation design from 2008 used bevel washers for anchor bolts bolting a pump to a channel skid, as the bolts were not perpendicular to the skid surface.

When to Use Bevel Washer

From AISC Guide to Design Criteria for Bolted and Riveted Joints, 2nd Ed P66

Below is from a pump foundation I designed in year 2008. I used bevel washer for pump anchor bolt bolting to
pump channel skid.
I was laid off back home after this project.
