WAIVER OF RIGHTS

I, ROGIE LAS PIÑAS FERNANDO, of legal age, Filipino citizen, married, and a
resident of Parola A, Gate 1, Tondo, Manila City, in accordance with law do hereby
depose and state that:

1. PARLYN HIPOLITO FERNANDO is my legal wife, being married with her on


January 10, 2011 at Quezon City Hall;

2. Due to personal reasons and irreconcilable differences, we decided to live


separately starting February 2012 up to the present;

3. My wife single-handedly took care of our minor child, ZHAILYN HIPOLITO


FERNANDO, and that she solely provides all their basic needs;

4. I hereby waive in favor of my wife PARLYN HIPOLITO FERNANDO all rights


and interest I may have over her provident benefits from the HDMF/Pag-IBIG Fund, and
other benefits in any public or private entities;

5. I hereby release and discharge the HDMF from any and all liability in
connection with the aforementioned waiver and release of the provident benefits in her
favor;

6. I am executing this affidavit to attest to the truth of the foregoing facts and
statements.
