Race competition held at Prk. Bless Brgy. Calumpang, General Santos City, March 2, 2019.
Acknowledge that have voluntariluy applied to participate in the following activities at the said
event.
I am aware that the above described activities are hazardous, and I am voluntarily participating in
these activities with the knowledge of the danger involved and agree to assume any and all risk of
bodily injury, death or property damage. I hereby agree that I, my assignees, heirs, distributes,
guardians, next of kin, spouse and legal representatives will not make a claim against, sue or
attached the property of the organizers for injury or damage resulting from the negligence or other
acts.
I have carefully read this agreement and fully understand it`s content. I am aware that this is a
release of liability and a contract between me and the organizers/sponsors/etc. And sign it of my
own free will.
