AFFIDAVIT

I, LORELEI JANE B. ALAMAG, of legal age, Filipino Citizen, and a resident of 40


Cashra Road Nivel Hills Lahug Cebu City, after having been duly sworn to in accordance
with law do hereby depose and states;

1. That I am working as medical analyst at Wipro BPO Philippines Ltd. Inc doing business
at Kasambagan Panagdait Mabolo Cebu City;

2. That to augment my income I sidelined selling food snacks to my co-workers in our


workplace;

3. That my co-employee Wilmarie T. Estallo gets food snacks from me which already
amounted to Nine Thousand One Eighty Five Pesos (P9, 185.00) accumulated from
January 2019 to February 2019;

4. That as the amount of food snacks she ate already accumulated to P9, 185.00, I
requested her to formalize her obligation by issuing a Promissory Note;

5. That in the Promissory Note that we signed, Wilmarie T. Estallo made it appeared that
her obligations she owed money from me in a form of loan which she promised to pay
on April 30, 2019 but in reality this is the value of the food snacks that she get from me
and still not paid;

6. That when the period falls due on April 30, 2019, I demanded her of the amount of
money which is P9, 185,00.00, however, she evaded me;

7. That I demanded her through text for the payment of her obligation, however, she
just ignored my text messages.

8. That until now, Wilmarie T. Estallo is evading my demand for payment and just
ignored my text messages, hence, I am executing this Affidavit as support of my
complaint for collection of sum of money against her.
