Special Instructions:
1. This permit shall be available at the work site at all times.
2. Location and description of work must be clearly indicated by the permit issuer.
3. All precautions given in permit must be strictly adhered to by the person to whom permit is
issued.
4. Permit shall be issued for maximum one day only. Extension shall be valid for one additional
day only.
5. After completion of work, used permits shall be preserved for record purpose up to 6 months.
