GENERAL INSTRUCTIONS:
PLEASE TAKE TIME READING THE GUIDELINES FOR THE PREPARATION OF DATA.
THIS WILL FACILITATE ACCURACY, CONSISTENCY AND ORGANIZATION OF ALL PRESENTATIONS.

1) Prepare a summary of your school's data on relevant Key Performance Indicators (KPIs) following the format in this tool.
2) On the upper portion, indicate the name of school, district, school head and public schools district supervisor.
3) Focus the data presentation on the 3rd quarterly themes, to wit: QUALITY, ACCESS, and GOVERNANCE.
4) Present the data using appropriate Key Stage Areas (KSAs). Leave blank any KSAs which are not applicable to your school cat
5) Identify the 3-year trend as I=Increasing; F=Fluctuating; and D=Decreasing.
6) Provide a brief description of factor/s that contributed to or facilitated the apparent trend.
7) Identify existing programs or projects in your school that are created to positively influence the current trend of specific K
8) Describe briefly the Impact of the programs or projects to specific KPI and/or to students and other stakeholders.
9) Cite and explain succinctly your proposed action/s in view of the KPI status, factors affecting specific KPI, and impact of imp
