This Monitoring and Evaluation Tool shall be accomplished by the SDO/RO SHS Focal
Person/s.
Objectives: To assess the implementation process on school’s SHS partnership building scheme
on the aspects of:
(a) determine whether the program is being implemented with fidelity;
(b) identify what planned resources and inputs were and whether they have came into being
(c) where relevant, assess whether desired outcomes are not be4ing achieved because of lack
resources or because of discrepancies between services that were planned and those that
are actually being implemented.
