AFFIDAVIT OF LOSS
I, RESHELL L. LLORICO, of legal age, Filipino, single, and a
resident of Brgy. Lapaz, Jamindan, Capiz, Philippines, after being sworn
in accordance with law, depose and say that:

1. I am a public school teacher and the holder of UMID No.


9570411091016.

2. I have kept my Unified-Multipurpose ID in my wallet.

3. On the first week of March, when I got back to our house after
travelling from Roxas City, I noticed that my UMID was missing
and until this moment cannot be found.

4. That after such discovery, I am exerted efforts in retrieving mu


UMID, however, the same proved futile and fruitless.

5. That I am executing this affidavit to attest all the foregoing


facts are true and correct based on my personal knowledge and
for other purposes it serve me best.
