Peace be with you!

I would like to sincerely apologize for not being able to attend our District and School-Based
In- Service Training due to the ff. unexpected occurrences:

1. My 74 years old mother had been brought to the hospital due to high blood pressure and
increased blood sugar. She had to be confined for immediate medication. Being her only
daughter, I had to attend to her needs during her stay in the hospital. She was confined for
four days starting May 21 to May 24 which were the first four days of our In-Service
Training as well.

2. Our 4 years old daughter was as well brought to the hospital right after my mother was
ready to be discharged due to severe stomach aches and extremely high fever and was
confined for 3 days dated May 24 – 26. She was diagnosed to have Amoebiasis.

I am very much willing to provide medical certificates for your confirmation. I am


hoping as well that my humble apology merits your favorable consideration. Thank you
and God bless!
