GENERAL INSTRUCTION

1. Candidates must paste THREE Passport size photographs attested by the concerned
Principal or his nominee at the spaces shown in the admission form.

2. Photocopy of DMC and REGISTRATION CARD duly attested by the concerned


Principal of his nominee must be attached with the admission form.

3. IN-SERVICE PERSONS must submit DEPARTMENTAL PERMISION, those failed


to furnish will be doing so at their own risk and responsibility.

4. Candidates are advised to submit admission form complete in all respect through their
respective Principals. Incomplete admission form will not be entertained.
5. Bank receipt in original must be pasted on the back side of the admission form.
