AFFIDAVIT
We, ROGIE FULGENCIO (a resident of Rizal, Diffun, Quirino) and
CKYZEL ANNE S. RINGOR ( a resident of Liwayway, Diffun, Quirino), both of
legal ages, Filipino citizens, after having been sworn to in accordance with
law, hereby depose and state that:

1. We are in a relationship for _____ years already;


2. Our relationship blessed us with our ___ year old child named
_________;
3. For years of being together, we never live under the same roof like
married couples do;
4. In fact, I (Ckyzel Anne S. Ringor) together with our child continued
to live with my parents in Liwayway, Diffun, Quirino;
5. On the other hand, I (Rogie Fulgencio) lives in Bayombong, Nueva
Vizcaya where I study and almost every weekend, I go home in my
parents’ house in Rizal, Diffun, Quirino;
6. I (Rogie Fulgencio) only get to see my child once a week whenever
I travel home to Quirino from Nueva Vizcaya.
7. Truth to tell, we are not living together nor are we planning to do
so not until we are legally married which will not happen anytime
soon because of our priorities;
8. We are voluntarily and freely executing this affidavit to attest to
the truthfulness of the foregoing facts and to prove that although
we have a child, we are not living together like husband and wife.

IN WITNESS WHEREOF, we have hereunto set our hands this 17th day
of April, 2019 in the City of Cauayan, Isabela, Philippines
