WAIVER AGREEMENT

I , Student of Bachelor of Science in Civil Engineering, hereby voluntarily


agree to join the site visit 2019-2020 on October 2 – October 4 at BAGUIO – PAGUDPUD – VIGAN which is
an academic requirement for the subject CVL 515 (SITE VISIT AND SEMINAR) with the following terms and
conditions:
 That I shall abide by all the rules and regulation and shall comply those imposed by the tour
especially concern with the safety.
 That I shall renounce and waive my claim against EARIST and any Professors for any injury that I
may sustain or loss that I may suffer, personal or pecuniary in the performance of my actions
while I’m in tour.
 Furthermore, the SUPERGRAND TOUR AGENCY will be liable to any injuries, accidents, and
damages from the beginning until the last day of the said tour.

September 15, 2019 with the amount of Php. 3,650 Kindly accomplish the reply slip below and return the
same to the assigned professor of your child.

In allowing my child to join this activity, I understand and acknowledge that Eulogio “Amang” Rodriguez
Institute of Science and Technology is not an insurer and not liable of the Participant’s or any party’s
behavior, such as personal injuries or property damages to participant/s or to third persons arising out of
the participation in the site visit or excursion. However, the Supergrand Tour Agency will be liable for the
said matter.
