Eduation Center. I understand that the school, the faculty and its staff and employees will exercise the due
diligence and prudence required for the safety and well-being for the duration, place, and his/her place, and his/her
participation for this event. This diligence would include oral and written instructions, whether given before or
during the activity that if followed, would ensure his/her safety. I also agree on the amount that will be required by
this activity which is thee Php200.00 and has the following breakdown:

Php180.00 - T-shirt and Printing


Php20.00 - Registration Fee
