JOINT-AFFIDAVIT OF COHABITATION

We, ERLINDA S. ICHON and MERIAM C. GRAN, both of legal age, married,
Filipino and a residents of Brgy. Punong, Tomas Oppus, Southern Leyte after
having been duly sworn to in accordance with law, hereby depose and say:

1. That we know the spouses DINAH B. MIRA and JAMES LOUIE P. BOHOL, as
husband and wife without the benefits of marriage since August 2008;

2. That the spouses lived in our Barangay since their cohabitation until the
death of JAMES LOUIE P. BOHOL. They were blessed with one child named
James Kyle Denise M. Bohol who was born December 13, 2015;

3. That the husband JAMES LOUIE P. BOHOL died on April 23, 2019;

4. That the widow DINAH B. MIRA is claiming the burial benefits of the late
JAMES LOUIE P. BOHOL with the DUNGGANON and RED CROSS as the
surviving spouse;

5. That we executed this affidavit freely and voluntarily in order to attest to


the truth and veracity of the foregoing and for whatever legal
intents/purposes this may serve.

IN WITNESS WHEREOF, We have hereunto set our hands this 30th day of April
2019 in the Municipality of Tomas Oppus, Southern Leyte, Philippines.
