I, EDGARDO B. LUMAYA, of legal age, Filipino, married, and a resident of 91


Cervantes Street Corner San Jose Extension, Dumaguete City, Oriental Negros after having been
duly sworn to in accordance with law do hereby depose and say;

1. I am a bona fide resident of 91 Cervantes Street Corner San Jose Extension,


Dumaguete City, Oriental Negros and was born on June 16, 1953 at Dumaguete City,
Negros Oriental;

2. I am at present not employed with any public or private firm or agency;

3. I am executing this affidavit to attest to the veracity of the foregoing statements, and
for all legal intents and purposes.
