Inspection report of Fahud Pipeline Project

Introduction including overview of area inspected and activities taking place

This report explains a workplace Health and Safety inspection carried out in Job # 833 Fahud Area
Gas Early Production System Pipeline Project of Oil & Gas on 20th June 2015 It is a pipe line project
for the fahood gas plant . The project area is almost 1.5 sq. km . The aim of the report is to highlight
good practices and areas requiring urgent action the worksites. The main areas of the site are plant
workshop and industrial yard, office, living accommodation and camp facilities.

The manpower engaged in all different regions accounted to 650 numbers having different trades as
welders, fitters, fabricator, electricians, plumbers, steel fixers, masons, scaffolders, helpers, managers,
engineers, supervisors, foreman, drivers and other supporting staffs. The major activity on site
consisted of hot work (welding, cutting, grinding), working at heights, excavation work, confined
space entry and carpentry work, working with power tools, manual handling and maintenance of plant
equipment The major activities in project are laying of concrete blocks, welding of pipelines and
installations of 32kv new overhead electrical lines. More than 45 percent completed in which the
activities are Mechanical and Civil works. In this project there are different types and plant equipment
including cranes , JCB , man lift bulldozers , generators . compressor are working . different types of
power tools and hand tools also using for the activities

Executive Summary
I have conducted a safety inspection in your esteemed organization to identify and recommend the
appropriate program and control to mitigate or reduce health and safety risk to as low as reasonably
practicable..
Based from the inspection and assessment in the said areas a good safety observation were noted such
as, Safety sign boards and formation sign boards safety induction to employees and non employees
before entering the site, and a good provision of relevant information to health and safety.
However, there are some serious concern such as issues related to, hot work activities, and hazardous
substance stored without MSDS and product label, excavation with out side protection ,not prepared to

meet the heat strees and reversing vehicles in the busy work area p,In this context a strict management
commitment required to review and HSE management system implementation by improved site
supervision, assign more competent HSE representative .Again management need to pay attention for
proper task specific training to the employees so that we can reduce the accident /incident which will
concrete your organization’s reputation and, social values and legal compliance. Analysis of the cost in
rectifying these problems as shown in the recommendation is relatively low working at the finds that
these branches could a threat. The conditions now represent a breach of C-155 , R-164 of the ILO.and
Oman labour law and Ministerial decisions and royal decrees also
The same time I have noticed some positive observations also such as adequate sign boards provided,
adequately maintained fire protection system, sufficient first aid materials and competent first aiders at
site etc.
For rectifying all these we have to spend some money, but when comparing the benefits what we are
getting by implanting is improved morale, good safety culture and improved production which all will
publicize your company’s reputation .Moreover will get an accident free smooth operation as well.

Main findings of the inspection


Main finding -1(hot work – welding work near the live sales gas area
It has been noticed that the welding work is going on near the live sales gas pipe line .this is doing
without gas test and with out covering the lives to avoid the accidental contact of sparks and arc to the
the line . besides to this gas monitor is not available in the site to check the presence of sales gas
leakage So totally we failed to follow the safety procedures here , if the welding is progress this this
manner there is a chance of huge and fire explosion and there property damage and fatality can
happen in the project . If an accident is happening it will invite the interference of Royal Oman police
and other agencies like fire department and factory directorate of Oman . company has to pay a huge
amount as fine and penalties . PDO may even terminate the contract of the company .Again property
damage and compensation to the employees will be very high. In addition to this ,By allowing workers
to work like this company is breaching provisions of ILO convention C-155 and recommendation –
R-164 which clearly states that it is the responsibly of the employer to provide a safe workplace and
safe equipment to the workers /In addition to ROYAL DECREE NO. 35/2003 is breaching by
keeping the workforce in dangerous situation
By providing a gas detector and covering the gas pipe line and by training to the employees we can
avoid the chance of this big accident . Cost for all these very less when comparing to the cost of an
accident. If company is providing all these, the result will be safe work place with a n accident free
operation of the project which will improve the morale of the employee , high production , away from
courts and other legal formalities and finally the company’s image also will be get improved
Main Finding 2-Safe work environment – Heat stress related
The climatic condition of the project area is very bad . Day by day the temperature and humidity is
increasing . But people are working in open area and company is not providing heat shelters and
proper drinking water facility .if company is dot preparing to meet the requirements sufficient facility
to the workers the result will be heat exhaustion and heat stress to the people. If employees are
suffering from heat stress ,naturally company production will be disturbed . in addition too this , the
heat related problem , especially the heat stroke can aggravated to a level of fatality . So it is company

is duty to provide sufficient drinking water and proper heat shelters .
As company is not providing safe work place company is breaching the general provision of Royal
Oman Ministerial Decision 153 dtd 12-10-2008 .& ROYAL DECREE NO. 35/2003 In addition the above
direction , company also breaching the general provision of ILO convention C- 155and
recommendation R-164By providing properly arranged drinking water stations , providing some
suitable rest shelters , and by conducting Heat stress refresher training company can give a safe work
place for the employees . by this action , you can avoid the fines penalty imposing by the Royal Oman
Police after the frequent site visit . moreover the safe work place will improve the morale of the
employee by that company will get improved production and totally your company image will be
improved among other PDO contractors

Main finding 3
Working in excavation without edge protection and keepin the material in the
edge
People are working in the excavation and there is sufficient side protection . besides to this some
excavated soil dumped inside of the excavation . This increase the chance of side collapse . if side is
collapsing the masons and other people working inside will be get buried and by that fatality of sever
injury will be the result if such accident is happening , the legal authority and Police will intervene
with the company activities and besides to this PDO also will go for investigation . in short it will
disrupt the company’s production . moreover company has to pay a huge amount as legal fines and
penalties , PDO can suspend your activities for some period
By allowing people to work in this condition company is breaching the general provision of IlO
convention –c-155and ILO recommendation R-164.Again it is clear violation of direction of Oman
labour law-&ROYAL DECREE NO. 35/2003 which all may invite the company for legal actions
By providing suitable side protection by sloping / benching . removing the excavated soil 1.5 mts away

excavation work . because of this company will get accident free safe production., by all these
comapnys image will be improved and less down time by improved morale of the employees

Main finding No 4 Lifting operations


lifting and rigging operation is one of high risk job in the site in the site rigging is going on without
competent rigger and with out barricading the area . again people are working under the suspended
load also . if the sling is failing or mistake of the rigging signal the material can fall from elevated area
or the crane itself can overturn to the people , resulting huge property damage and fatality to the
people. If such accident is happening the PDO will stop the work and other legal agencies wil
interfere with the investigation of the accident finally company has to big amount as penalities and if
fatality occurs company management will even get jail punishment also
by allowing people to work like this and operating the the crane without competent crew is a clear
breach of ILO convention-155 and recommendation R_164 . again it is breaching the general
provisions of ROYAL DECREE NO. 35/2003
By hiring a competent rigger and by efficient supervision company can get a safe crane operation.

Again safe rigging and lifting training need to conduct with all the lifting crew , sothat while lifting
area wil be barricaded and signage also will be provided so that no body will work under the
suspended load
By giving all these company is getting a accident free production , reduced disruption n to the normal
duty and happy employees . Thereby company can attain the production target , and be away from
fines and other formalities
Main Finding 5 Vehicle operations – reversing the vehicle
Noticing that vehicles are reversing in the site where people are working Ina addition to this there is
no back up alarm with the trailers . if vehicles are reversing in the site without back alarm chances of
collision with different group of people working there or otherwise it may collide with the other
moving equipment or static property ,the accident by this can aggravate to a level of fatality or severe
property damage , Both will be big impact to the company . If a vehicle accident happening the Royal
Oman police will interfere with the work and if there is a sever injury or fatality happen , legal
agencies will go for investigation and further proceeding . it can end up with fine or imprisonment
resulting it is disrupting the normal function of the company .
By allowing like this and putting the life of people in danger , it is a breach of Oman Labor law and
other provisions of ILO convention C155 and recommendation R-164 which inviting legal actions
aginst the company
By providing a specially designated place for reversing th vehicle , or fitting a sensing device, fitting a
new back up alarm company can reduce the chance of accident from vehicular operations. The cost for
rectification will be less when comparing to the work disruption and other expenses spending for the
legal and other formalities . besides to this by implementing all these companies accident statistic wll
become less which will cause less insurance premiums and improved production
Main finding 6 Chemical Hazard – NO MSDS AVAILABLE IN HT ECHEMICAL
STORE
As per COSHH regulation , when chemical are using the MSDS should be provide with each chemical
. in the store ther are a lot of chemicals but the MSDS is not available . MSDS is giving a general idea
about he safe use of chemicals and storage requirements and disposal also . All the hazards are clearly
mentioned in it .If there is no MSDS present with the chemical people can identify the hazards of the
chemical s and by mistakly without taking precautions it will enter to their body
If a hazardous chemical is entering to the body the chemical effect will harm to the individual . .
Again Non availability of MSDs is a it is breach of general provisions of COSHH rule and general
provision of ILO recommendations R-164
The company can require the supplier to provide the MSDS with the chemical or otherwise can down
load it from internet also . By providing MSDS , workers will follow the safety requirements
mentioned in the MSDS and t will reduce the chance ill effects from the hazards of the chemicals . this
will give more production and will reduce the hospital and other medical expenses and unwanted
absenteeism of the employees . In short by supplying MSDS will give a better job performance and
improved production

Conclusion
A good number of positive and negative things are noticed in the site during my inspection . The main
issues of concern are hot works activities especially which are carrying out near the live hydrocarbon
line , employees are working in open area and there is no proper provision for heat stress shelters and
drinking water facility, excavation without side protection and dumping the excavated soil near to the
excavation edges , lifting activity without trained rigger and people are working under the suspended
load ,reversing the vehicle in the area where people are working and MSDS not available in the store
All these above mentioned unsafe practice has a potential to cause major accident in the site and if an
accident is happening g, the companies progress will be get affected and a reputed company like
yours , its image also will be get affected and thereby the share value will get down . As a common
because of an accident , legal implication can happenand huge amount of time and money need spend
for legal matters and accident investigation . By a major accident companies insurance premium also
will be get increased
But by allocating some resources spending some money company can maintain its good health and
safety standard and it will improve the image among your competitors also . The main
