D : Good afternoon, may I help you?
P : good afternoon doc, yes you can help me?
D : yes, how do you fell?
P : I fell im not better because my back very pain doc.
D : from number one to number ten, how the intensity of a pain?
P : I thing six doc and they are very annoying!
D : how long if you been feeling a pain?
P : there month after the accident doc.
D : what happened to you?
P : car accident doc. I wasn’t paying attention and I hit a car that stopped in front of me.
D : ok, let me check you.
P : what happened to me doc?
D : my diagnosis says that your condition’s is not normal so i will give you prescribe
painkiller for your pain and I would like you to rest for the time being. This is you
prescription and you can get it drug in the drug store.
P : ok, thank you doc
D : your welcome and I hope you get well very soon.

D : how are you ?
P : I fell not better doc
D : did you take your medicine?
P : yes doc but I can fell a pain again after one hours to take a drug.
D : ok, I will give you referall to doctor chiropractor for your detailed check up in hospital.

DS : ya doctor jiszie, what happened?
D : this is patient ms. Febriana and this is her medical report. Her diagnosis is pain in back .
DS : ok thank you for your information doctor jiszie
D : your welcome doctor bella. Ms febriana she is doctor chiropractor she name is doctor
bella . doctor bella I will back.
DS : ok, doctor jiszie be carefull and thank you.
D : oke.
DS : ms bella can you tell me what happened to you?
P : yes doc, I fell im not better because my back very pain doc.
DS : ok, I see your medical report and my diagnosis say that your condition’s is not
normaly!. Because after you take drug you can’t better.
P : what should I do doc?
Ds : you can need some treatment in the hospital ms. Febriana.
P : ok doc please make it better.
D : yes ms. Febriana

DS : nurss please come in.
N : yes doc, what happened?
D : please check up her
N : exuse ms. Febriana let me check you
P : yes nurse
N : doc after I checked , body temperature thirty eight degrees celcius, blood pressure
sistole one hundred twenty and diastole eighty, pulse eigty, and respiratory rate fifteen
in one minute.
D : thank you nurse.
N : your welcome doc.
D : ms. Febriana I can diagnose that your healthy is not good, your body temmerature is
still high. You need to have more rest to make it better.
P : ok doc
D : nurse please you take patient to her ward
N : yes doc, ms. Febriana please follow me
P : yes nurse and thank you doc
D : your welcome ms. febriana
*STAGE 4

N : good morning ms. Febriana.


P : good morning to nurse. what about good news nurse?
FP : good morning to nurse, I am a patient family, my name is grascella and I am mother of
febriana
N : nice to meet you ms. Gracella
FP : nice to meet you to nurse
N : ms. Grascella and ms. febriana I have good news for you!
FP : what about good news nurse?
N : ya ms. Grascella, ms. Febriana get recovered and allowed to go home.
FP : are you suer nurse?
N : yes, and I give you manner to take care of herself at home and don’t forget to take
some exercise and always positive thinking.
FP : thank you very much nurse
N : your welcome and I hope you can run a better life ms. Febriana.
