Affidavit of Loss

I, MARIA AGWANTA , Filipino, of legal age, single , and a resident of Campao Oriental,
Getafe, Bohol, Philippines, after being duly sworn to in accordance with law, hereby
depose and state:

1. That I am former student of HALLOWEEN COLLEGE, Tubigon, Bohol during the Second
Semester of SY 2016-17 as a Second Year BSC-MA student and as such I was issued a
SCHOOL IDENTIFICATION CARD;

2. That I was required to surrender my school ID as a requirement for issuance of my


requested Transcript of Records and Honorable Dismissal ;

3. Thereafter, I searched for the said ID , however, the same could not be found
anymore;

4. That despite diligent search and efforts to locate my said HC School ID Card, I could
not find the same such that I now believe that it is now lost beyond recovery;

5.As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing and
for whatever legal purposes this affidavit may serve.
