I, DIGNA LEDESMA PAGAL of legal age, married ,Filipino, and a resident of


Nangalisan, Solana, Cagayan, after having been sworn, hereby depose and state:

1. That I secured a copy of my Birthcertificate at the PHILIPPINE STATISTICS


AUTHORITY(PSA) my middle name was written as “LADESMA”;
2. That however, in all my records and Valid ID’s, my middle name was written
as “LEDESMA”.

3. That I intend to follow the correct name as indicated in my PSA Birth Certificate.

4. As such, I am executing this Affidavit of Discrepancy to attest to the truth of the


foregoing .
