AFFIDAVIT OF DESISTANCE

I, AGNES ABELLA Y NUÑEZ, of legal age, single and resident of 9 th Street, 10th
Avenue, Caloocan City after having been duly sworn to in accordance with law, depose
and say:

1. That I am the complainant against ELICARL OTILLA Y SORIANO for Physical


Injuries in relation to Republic Act 7262 or Violence Against Women and Their
Children Act now pending with Branch _______ Regional Trial Court of Quezon
City;

2. That after self-examination and evaluation of the facts and incidents surrounding
the case which led to its filing I have realized that all were due to
misunderstanding between us as live-in partners and still love him hence, I
decided to forgive said respondent;

3. That I am no longer interested in pursuing further the case and withdraw the
same against my live-in partner Elicarl Otilla y Soriano and hereby desist from
the case;

4. That am fully aware of the consequences in the execution of this desistance and
requests the authorities concerned to dismiss the case against the above-named
respondent;
