JOINT-AFFIDAVIT OF TWO (2)
DISINTERESTED PERSONS
WE, CHARLES CONRAD S. COCAMAS and LINDANITA D. TAPONG, both of
legal ages, Filipinos, married, and residents of Brgy. Tibal-og, Sto. Tomas, Davao del
Norte, Philippines, after having been sworn to in accordance with law, do hereby
depose and say that:
1. We are the same affiants who personally and voluntarily caused the
preparation and execution of this joint-affidavit;
2. We personally know the spouses Renito Agor Batoc and Greciela Piñapil
Unabia-Batoc, who contracted marriage on October 29, 1990 at MCTC
Carmen, Sto. Tomas, Davao del Norte, Philippines;
3. During their union, they were blessed with five (5) children. Three of which
are CHARMEN UNABIA BATOC-GEOLAMEN, CHERRYMEE UNABIA
BATOC-CARABEO, and CLIFFORD UNABIA BATOC;
4. Upon verification of CHARMEN UNABIA BATOC-GEOLAMEN’s Certificate of
Live Birth, it appears that her middle name was erroneously recorded as
ONABIA, instead of UNABIA, which is her true and correct middle name.
Likewise, it also appears that the name of her mother was incorrectly
recorded as GRESILDA REPEL ONABIA, instead of GRECIELA PIÑAPIL
UNABIA, which is the true, correct and complete name of her mother.
Moreover, it also appears that the date of her parent’s union of marriage
was not indicated instead of OCTOBER 29, 1990, which is the true and
correct date of her parent’s marriage;
5. Upon verification of CHERRYMEE UNABIA BATOC-CARABEO’s Certificate
of Live Birth, it appears that the middle name of her mother was
erroneously recorded as PIÑAFIEL, instead of PIÑAPIL, which is the true
and correct middle name of her mother. Likewise, it also appears that the
first name of her father was incorrectly recorded as RENIE, instead of
RENITO, which is the true and correct first name of her father. Moreover, it
also appears that the date of her parent’s union of marriage was not
indicated instead of OCTOBER 29, 1990, which is the true and correct date
of her parent’s marriage. Lastly, it also appears that her birth order was
mistakenly recorded as FIRST (1st), instead of SECOND (2nd), which is her
true and correct order of birth;
6. Upon verification of CLIFFORD UNABIA BATOC’s Certificate of Live Birth, it
appears that the middle name of his mother was erroneously recorded as
PENAFIEL, instead of PIÑAPIL, which is the true and correct middle name
of his mother;
7. It bears stressing that the aforementioned erroneous entries were made due
to the negligence and inadvertence of the aforementioned spouses in

verifying the true and correct information needed during the registration
process before filing the same;
8. Being our long-time friends and neighbors, we declare the fact that the true,
correct and complete name of their mother is GRECIELA PIÑAPIL UNABIA-
BATOC and that the names GRESILDA REPEL ONABIA-BATOC and
GRECIELA PIÑAPIL UNABIA-BATOC, refers to one and the same person.
Also, we likewise declare the fact that the true, correct and complete name
of one of the abovementioned sibling is CHARMEN UNABIA BATOC-
GEOLAMEN, and that the names CHARMEN ONABIA BATOC-GEOLAMEN
and CHARMEN UNABIA BATOC-GEOLAMEN, refers to one and the same
person. Moreover, we also declare the fact that their parent’s date of
marriage is OCTOBER 29, 1990. Furthermore, we likewise declare the fact
that the true, correct and complete name of their father is RENITO AGOR
BATOC, and that the names RONIE AGOR BATOC and RENIE AGOR
BATOC, refers to one and the same person. Lastly, we further declare that
CHERRYMEE UNABIA BATOC-CARABEO’s order of birth is SECOND (2nd)
and NOT FIRST (1st).
9. We are not related to the aforementioned persons either by affinity or
consanguinity;
10. We are executing this joint-affidavit to attest to the veracity of the
foregoing declarations, to apprise the authorities concerned and for
whatever legal purpose this may serve best.
IN WITNESS WHEREOF, we have hereunto affixed our signatures this July 17,
2019 at Sto. Tomas, Davao del Norte, Philippines.
