TERMS & CONDITIONS SUPPLIER COPY
1. This Pass is issued by the Gate Guard-on-Duty upon entry to the the MGCVHAI Clubhouse.
2. The requesting party should be responsible in providing clear description of personal item/s, equipment (e.g.
instruments, cooking utensils, electronic gadgets) that is/are brought into the Clubhouse.
3. This Pass serves as Exit Pass for the equipment and should be kept for security reference.
4. In case of loss of this Pass, the Security Office should be notified immediately.
5. Only the item(s)/equipment listed herein will be allowed by the Guard to be brought out of the venue.
6. Undeclared items will be withheld at the Security Gate until a clearance from the person-in- charge of the activity
has been secured.
