DIVERSIFIED TECHNIQUE
CHIROPRACTIC TECHNIQUES
Diversified Technique is the classic chiropractic technique, developed by D.D. Palmer, DC.
and taught in all chiropractic colleges. Diversified Technique was refined and developed by
the late Otto Reinert, DC, to address biomechanical failure in each section of the spine, as it
relates to specific subluxation. The focus is on restoration to normal biomechanical function,
and correction of subluxation. In addition, Diversified methods have been developed to
adjust extremity joints, allowing for beneficial applications in treating sports injuries and
other injuries. Diversified adjusting of the spine uses specific lines of drives for all manual
thrusts, allowing for specificity in correcting mechanical distortions of the spine. X-rays and
case histories are used in analysis and diagnosis. No instruments are used in the adjusting
procedure. Motion palpitation and full spine, hands-on techniques are used to deliver a
deeper thrust, which makes an osseous (popping) sound as the adjustment is given. Click

FLEXION-DISTRACTION
Spinal Decompression

Dr. Cox explains Flexion-Distraction (also known as Cox Technique), as a marriage of


chiropractic principles with the osteopathic principles set forth by Alan Stoddard, DO, in his
book, Manual of Osteopathic Technique, written about the manipulative procedures
developed by John McManis, DO, in the early 1900’s. Since the early 1970’s, Dr. Cox has
refined the technique, developed a manipulation instrument for effective use of the
technique, conducted clinical, as well as participated in experimental, research; lectured
around the world, and written well-received articles, chapters for textbooks, and textbooks.

THOMPSON DROP TECHNIQUE


Thompson Terminal Point Technique is built around the research of Dr. Derifield of Detroit
Michigan . Dr. Derifield noticed that the same adjustment on two patients with the same
analysis did not always give the same results. As he gained more experience, he found that
the answer lay in determining through analysis whether the subluxation was lower spine or
cervical. Analysis of the patient’s subluxations, using the Derifield Technique, depends upon
a very accurate leg check. Dr. Derifield created a set of rules for checking the legs to
determine the point of the subluxation in the pelvic and lumbar area. It was further
determined that turning the head in certain ways helped to balance the legs. Later, Dr. Niblo
added testing various trigger points on the side of the short leg, and adjusting the anterior
ischium, producing astonishing results. Using a Thompson terminal point table, which is
especially designed for the Derifield technique, a special weighing mechanism in the
terminal point table instantly weights the patient and adds only enough tension to hold the
patient in the “up” position before the thrust is given.

ACTIVATOR METHODS
Adjuster

Activator Methods is based on the postulate that understanding body mechanics and force
is the key to understanding how to move bones. A leg test is utilized to tell the doctor if the
subluxation exists, chiefly in the lumbo-sacral area or cervical region of the spine. With
further research into body mechanics, W. C. Lee D.C. and A. W. Fuhr D.C. were able to
analyze the body and produce from the analysis consistent changes using a light non-force
specific chiropractic adjustment. Lee and Fuhr understood that force was not necessary to
correct subluxations in the body. Two components are necessary for fast, effective
reduction of nerve pressure caused by subluxations. They are speed and line of drive.
Speed and line of drive are what make chiropractic the distinct and dynamic art and science
that it is. The activator instrument was designed to deliver this specific dynamic thrust. It is a
small hand-held gun-type mechanism that delivers a sharp percussive thrust upon
triggering. The activator instrument controls the force and speed of the adjustment
exceptionally well and with the activator, the line of drive can be more specific.

What Treatments Do Chiropractors Offer?

If you've ever seen a character on a TV show or movie receive chiropractic care, you
may assume that spinal manipulations are the only treatments chiropractors offer.
During spinal manipulation, your chiropractor uses quick, hands-on pressure to realign
the small vertebrae in your spinal column. Many people refer to this as "cracking the
neck," but the cracking sounds produced during manipulation are actually gas bubbles
released from the joints.

Improving the alignment of the spine with spinal manipulation reduces inflammation,
improves flexibility and range of motion, enhances blood flow and decreases pressure
on the joints, nerves, muscles, and ligaments.

In 2017, the Journal of the American Medical Association published a systematic review
of studies that evaluated the effectiveness of spinal manipulation in treating acute low
back pain. The review noted that manipulation was associated with less pain and
improved function six weeks after treatment.

Other studies have shown spinal manipulation and chiropractic treatments to be


effective in treating whiplash, neck pain, plantar fasciitis, hip and knee osteoarthritis,
migraines and other conditions.

In addition to spinal manipulation, your chiropractor may also offer these treatments:

 Massage. Massage is often used in conjunction with chiropractic treatments and may
be helpful in decreasing pain, easing muscle tension and reducing stress. Massage
increases blood flow to the injured area and promotes healing.
 Soft Tissue Mobilization. Mobilization involves gently moving and stretching the soft

 Transcutaneous Electrical Stimulation (TENS). TENS treatment uses a gentle, low-
voltage electrical current that blocks pain signals.

diagnostic tests, and discuss your medical history and symptoms before recommending

Chiropractic Treatment May Relieve Your Vertigo

It's hard to focus on anything when the room feels as if it's constantly spinning due to
vertigo. The condition affects nearly 40 percent of us at some point in our lives,
according to University of California San Francisco Health. If vertigo has disrupted your
life, chiropractic treatment may offer the relief you desperately need.

When You Can't Get Off the Merry-Go-Round

Vertigo can make you feel as if you're on an amusement park ride that never ends. In
addition to the spinning sensation, you may also experience dizziness, nausea,
vomiting, difficulty walking and speaking, headaches, sweating, and jerky eye
movements.

Medications can stop or reduce nausea and vomiting and help you feel less dizzy, but
your symptoms may return if you stop taking the medication. Chiropractic treatment
targets the source of your problem without the use of medication.

Common Causes of Vertigo

Vertigo can be caused by:

 Injuries. You may first notice vertigo after a head or neck injury.
 Misalignments and Dislocations. Vertigo may be more likely to occur if your spine
isn't aligned properly or your vertebrae are dislocated.
 Inner Ear Problems. Your inner ear plays an important role in balance. Infections and
conditions that affect the inner ear can be the cause of vertigo.
 Meniere's Disease. The disease causes vertigo, ringing in your ears and hearing loss.
 Benign Paroxysmal Positional Vertigo (BPPV). BPPV can be caused by aging or
head trauma and occurs when calcium carbonate crystals in the inner ear become
dislodged and enter the semicircular canals deep inside your ears. The crystals interfere
with the normal movement of fluid in the canals, triggering vertigo symptoms.
 Your Habits. Vertigo can sometimes occur if your diet is unhealthy or you consume or
use caffeine, alcohol or other substances that affect the nervous system.

A Visit to the Chiropractor Can Improve Your Symptoms

Chiropractors offer several treatments that may relieve your vertigo. If you've injured
your neck or head or have a spinal misalignment, a chiropractic adjustment may ease
your symptoms. During the adjustment, your doctor uses his hands or a small
instrument to gently realign your spine. If the joints in your neck don't move properly,
your brain may misinterpret the information it receives from your body. Adjustments can
improve movement in joints and reduce vertigo.

confirmed that spinal adjustments can be helpful in treating vertigo symptoms. After
chiropractic treatment for 13 months, most subjects noticed improved balance. Less
dizziness and neck pain were also reported.

moves your body through a series of quick position changes that take advantage of the

How to Extend the Effects of Your Chiropractic Adjustment

Depending on the things you do after your visit to the chiropractor, the effects of your
treatment can last days or just a few hours. Fortunately, you can extend the positive
effects of your adjustments by following a few simple guidelines.

Pay Attention to Your Posture

Poor posture can contribute to spinal misalignments, tight muscles and other
uncomfortable symptoms. Although you may feel better after the adjustment, your
results won't last very long if you slouch or spend hours hunched over your laptop or
phone. When you use your phone or another digital device, hold it at eye level if
possible. Constantly looking down at devices puts enormous strain on your neck and
may even cause permanent damage to your spine if you don't change your habits.

Use Ergonomic Furniture

Slouching may be unavoidable if your chair and desk force your body into an
uncomfortable position. Ergonomic chairs keep your spine properly aligned, support
your lower back, and distribute your weight evenly. When they're combined with
ergonomic desks and keyboards, you may be much less likely to experience aches and
pains at the end of the day.

Standing desks offer another ergonomic option. Using a standing desk several times
throughout the day can reduce pain in your upper back and neck and may improve your
productivity. The desks are adjustable and move up and down to allow you to move
easily from a seated to a standing position.

Take Regular Breaks

Improving your posture and using ergonomic furniture can help you keep your spine
properly aligned, but pain may still be a problem if you spend long hours at your desk.
Add frequent breaks to your daily schedule to prevent pain and discomfort. Take a short
walk (even if it's only to the end of the hall) and do a few stretches before you return to
your desk.
Avoid Awkward Movements or Lifting

Activities that involve awkward movements or require you to contort your body can
cause aches and pains. Painting the ceiling or digging new posts for your fence the day
after your treatment can reverse the results of your adjustment.

Poor lifting techniques can also ruin the effects of chiropractic adjustments. Don't be
afraid to ask for help and use your knees and legs, rather than your back, when you lift.

Fill Your Water Bottle

Water helps your body get rid of toxins that are released after an adjustment. Increasing
your water intake for a few days after your treatment can be very beneficial. Water
improves joint lubrication, which may help keep your joints become more flexible.
Upping your water intake can also help you avoid dehydration, a condition that can
cause headaches, muscle weakness, dizziness, fatigue, and irritability.

Get a Little Exercise

Although you won't want to participate in strenuous activities soon after an adjustment,
low-impact exercise will help keep your muscles loose and improve your range of
motion. Walking, swimming, biking, yoga, Pilates and other forms of exercise will
enhance your adjustment results.

Your chiropractor may have taught you a few exercises you can do at home to improve
the strength and flexibility of your muscles. These exercises are an important part of
your treatment plan and should be performed as often as your chiropractor
recommends.

Don't Skimp on Sleep

Sleep plays a pivotal role in the healing process. While you're sleeping, your body
repairs muscles and tissues and produces hormones that reduce inflammation. If you
don't get seven to eight hours of sleep every night, you may be unwittingly sabotaging
the effects of your chiropractic treatment.

Do you sleep on your back? This position may be a factor in your pain. The American
Chiropractic Association notes that you put approximately 50 pounds of pressure on
your back when you sleep on it.

The 33 bony vertebrae that make up your spinal column not only support your weight
and allow you to stand upright but also protect your spinal cord. The cord passes
through small openings in the vertebrae called the vertebral foramen.

Nerves attached to the spinal cord exit the vertebrae at multiple points along the spinal
column and branch off to organs and other parts of the body. The complex interaction
between your spinal cord, brain, nerves, and nerve endings ensures that your body
functions optimally. The nervous system controls many of the body's most important
functions, including temperature regulation, heart rate, vision, digestion, pain sensation,
and movement.

If the spinal column becomes misaligned, vertebrae may begin to press on the nerves,

Many people of all ages experience depression and/or anxiety, whether on an


occasional, frequent, or consistent basis. These symptoms may be transient, only
occurring at a particular stage in one's life, or they may represent a specific condition
such as a depressive disorder or a panic disorder.

Panic disorders are characterized by attacks of anxiety and overwhelming fear known
as panic attacks. During a panic attack, a person may experience pounding or racing of
the heart, breathing problems, dizziness, weakness, chills, and nausea. Additionally,
during a panic attack a person may experience a feeling of being out of control and a
feeling of impending doom.

Chiropractic care has a distinct and specific role to play in the management of panic
attacks. Spinal bio mechanical dysfunction and spinal nerve irritation may be implicated
in the causation of panic attacks, as such dysfunction and irritation can heighten
physiological stress and participate in triggering feelings of anxiety. In other words, if
your body is already on high-alert status owing to internal bio mechanical stress and

specific motion using my hands or an instrument to restore motion to the joints. This motion
is known as an adjustment. I have been trained in Diversified, Thompson Drop Table,
Gonstead, SOT, Flexion Distraction, and Biophysics techniques. This just means I can
adjust you in a lot of different ways. The adjustments that I administer can be low or high
force, depending on what you need and your comfort level. Some adjustments are
accompanied by a characteristic popping noise. Most of my patients describe adjustments
as comfortable or even relaxing.

They usually graduate with a pre-med major after having taken courses in

They then attend a chiropractic graduate program. On average, these involve


4 years of education with a total of 4,200 instructional hours in course credits.
Chiropractic program specifics
Divided by year, a chiropractic graduate program usually involves:

 First year: Courses in general anatomy, chiropractic principles,


biochemistry, spinal anatomy.

 Second year: Courses in chiropractic procedures, pathology, clinical

 Third year: Courses in clinical internships, integrated chiropractic,

Several myths surround this question. One myth is that chiropractors only
treat back pain. In fact, chiropractic care can also help to heal pain in the foot,
elbow, shoulder, and neck.

The same review cited by the National Center for Complementary and
Integrative Health concluded that chiropractic treatment is not useful in
treating:

 asthma

 hypertension

 menstrual pain

Authors of the review failed to find definitive evidence that chiropractic care
treated musculoskeletal conditions, such as fibromyalgia, temporomandibular
joint disorders, and mid-back pain.

Chiropractors also do not treat traumatic injuries, such as bone fractures. A


chiropractor will usually perform an X-ray to ensure that treatment will not
worsen a traumatic injury.
Science supporting chiropractic
treatment
Studies suggest that chiropractic methods are viable options for managing pain.

A 2018 review included 17 years of studies involving spinal manipulation and


mobilization, which is a more passive form of manipulation.

The studies investigated the effects of these treatments on chronic lower back
pain, and the authors concluded that the chiropractic methods were "viable"
options for pain management.

A 2017 review examined the effectiveness of spinal manipulation in treating


lower back pain.

The authors concluded that treatment improved both function and pain for up
to 6 weeks.

The American College of Physicians recommend that those with lower back
pain use a variety of non-pharmacological treatments, including spinal
manipulation.

Researchers generally agree that more studies are needed to determine the
ideal length and frequency of chiropractic sessions and to identify what
injuries may benefit from specific treatments.

Is it safe?
One of the most common sources of contention regarding chiropractic
treatments concerns safety.

A person may experience side effects of spinal manipulation, including:

HISTORY OF CHIROPRACTIC
Manual manipulation of the spine and other joints in the body have been around for a long
time. Ancient writings from China and Greece dating between 2700 B.C. and 1500 B.C.
mention spinal manipulation and the maneuvering of the lower extremities to ease low back
pain. In fact, Hippocrates, the famous Greek physician who lived from 460 to 357 B.C.,
published a text detailing the importance of manual manipulation. In one of his writings he
declares, “Get knowledge of the spine, for this is the requisite for many diseases”. Evidence
of manual manipulation of the body has been found among the ancient civilizations of
Egypt, Babylon, Syria, Japan, the Incas, Mayans and Native Americans.
The official beginning of the chiropractic profession dates back to 1895 when Daniel David
Palmer restored the hearing of Harvey Lillard by manually adjusting his neck and something
told him that he was into something good. Two years later, in 1897, Dr. Palmer went on to
begin the Palmer School of Chiropractic in Davenport, Iowa, which continues to train
doctors of chiropractic to this day.
Throughout the twentieth century, the profession of chiropractic has gained considerable
recognition and scientific support. Research studies that have clearly demonstrated the
value of chiropractic care in reducing health care costs, improving recovery rates and
increasing patient satisfaction. In fact, one very large study conducted in Canada, the 1993
Manga Study, concluded that chiropractic care would save hundreds of millions of dollars
annually in work disability payments and direct health care costs. Several major studies
conducted by the U.S. Government, the Rand Corporation and others, have all

If you spend the majority of your day sitting in front of a computer, you may have
experienced the tell-tale signs of postural strain – colloquially known as student or office
worker syndrome. Back pain, shoulder pain and a sore neck can all be indicators of
spending too much time hunched over a desk.

In honour of World Spine Day on Oct. 16, Saanich chiropractor Mark Strudwick sat
down to discuss spine health.

Strudwick, who has been practicing in Greater Victoria since 1998, sees folks with
postural strain all the time. It started with government employees and now it’s folks from
all professions and of varying ages, he noted.

Sitting in a hunched position while working on a laptop or desktop for prolonged periods
of time can result in pain in various parts of the spine as the neck and shoulder muscles
curve. Studies show that almost half of office workers experience neck pain caused by
poor posture at some point. Looking up can compensate for this as it straightens the
neck and back, Strudwick explained, so moving your monitor to eye level is beneficial.

Strudwick also recommends sitting upright with the arm, knee and hip joints at 90
degrees. Standing desks can be a good solution for back pain too, he noted.

While sitting ergonomically at work can relieve postural stress, it’s best to avoid working
on a computer for any longer than you need to, so Strudwick suggests staying off
computers outside of work hours if possible.
The theme for World Spine Day 2019 is “Get spine active!” Folks are encouraged to
think of their spines and find ways manage back and neck pain – it can be as simple as
taking a stretch break every 20 to 30 minutes at work, going for a walk or seeing a
chiropractor, said Strudwick.

a chiropractor, and pediatricians say it’s okay.

According to Dr. Daniel Erickson in Rochester, adjustments on infants has a


place as long as it doesn’t replace traditional medicine.

One of the common conditions chiropractors are able to help in infants is


colic, or excessive crying.

“We see really good results with just one to five treatments with an infant.”

Dr. Erickson said parents should always consult their child’s pediatrician

adults experience pain most days or every day.
Back pain, neck pain, and headaches are three of the most

the United States, it’s also the one doctors struggle the most to
treat. Prescription pain pills, including opioids, tend to provide
inconsistent and often diminishing relief for people who
experience chronic pain. Some of these drugs also come with
significant risks for addiction, withdrawal, and other dangerous
side effects. For these reasons, many national health authorities
have begun to recommend non-pharmacological treatment
options for common pain conditions. The American College of
Physicians, one of the largest physician groups in the United
States, now advises its members to treat most forms of low-back
pain with massage, acupuncture, and other non-drug
interventions. Among these, chiropractic spinal manipulation is
one of the most widely used and research-supported forms of
“alternative” pain therapy.

A central tenet of chiropractic medicine has always


been that improper spine alignment is the root cause of many
musculoskeletal ailments. By correcting misalignments, which
some chiropractors call “subluxations,” spinal manipulation can
purportedly treat a range of health complaints. The National
Center for Complementary and Integrative Health (NCCIH),
which is a branch of the National Institutes of Health, says that
chiropractic “emphasizes the body’s ability to heal itself.”

Chiropractic was founded toward the end of the 19th century by a


health enthusiast in Iowa with no formal medical training. It was
controversial from the start, and it was widely derided for much of
the 20th century in part because some of its early practitioners
rejected the practice of immunization and refuted germ theory, the
science-backed belief that bacteria, viruses, and other pathogenic
microorganisms cause certain illnesses. While specific practices
and philosophies vary widely among chiropractors today, most
have abandoned those early and contentious positions in favor of
evidence-backed approaches.

“Chiropractic was the original holistic health-care profession,”


says Michael Schneider, a licensed chiropractor and associate
professor in the Department of Physical Therapy at the University
of Pittsburgh. “A deeply held belief among chiropractors is that
you need to treat the whole person, not just the body part that
hurts.”
This mosaic of treatment approaches can make
it hard to pin down chiropractic’s risks and
benefits.

Schneider says there’s good evidence supporting the use of


chiropractic spinal manipulation for low-back and neck pain, and
that it can also help treat conditions that stem from
musculoskeletal issues, such as nerve pain and some types of
headaches.
A 2017 research review in JAMA found that spinal manipulation
was associated with modest improvements among people with
acute low-back pain. While “modest” may not sound like a ringing
endorsement, an editorial accompanying that JAMA review points
out that when pitted against “conventional” medical care —
namely, drugs or surgery — spinal manipulation works at least as
well and comes with fewer risks. Also, a 2013
survey from Consumer Reports found that among patients with
low-back pain, treatment satisfaction rates were almost twice as
high for those who visited a chiropractor compared to those who
met with a primary care physician.

The NCCIH says there’s evidence that spinal manipulation may be


as helpful as or more helpful than drugs for the treatment of neck
pain and migraine headaches, and that it may also help relieve
sciatic nerve pain. The NCCIH also says spinal manipulation is
safe when performed by a licensed chiropractor; while it’s
common for people to experience muscle soreness or stiffness, or
even a temporary increase in pain, serious side effects are “very
rare,” the NCCIH states.

B ut a big question lingers: How does spinal


manipulation work? One theory, Schneider says, is that adjusting
the spine stimulates nerve activity in ways that change how the
body perceives pain. “We’ve measured what happens to spinal
joints during manipulation, and we do see a gaping of joints,” he
says. This gaping may trigger “a very strong neurological reaction
in the body,” which may result in the observed pain benefits, he
adds. But all these findings are preliminary and require more
study.
Some have argued that chiropractic’s benefits are attributable to
the placebo effect. A 2016 study in the Journal of Chiropractic
Medicine found some evidence, albeit inconsistent, that spinal
manipulation outperformed a sham procedure for the treatment of
low-back pain. But as with acupuncture and other alternative pain
treatments, it’s difficult for researchers to design a convincing
sham alternative to chiropractic spinal manipulation. This makes
it tough for them to rule out the placebo effect. “We should be
cautious of making strong inferences based on these results,” says
Rachel Perry, first author of the study and a senior researcher at
the University of Bristol in the U.K.

Also complicating matters is the fact that specific techniques and


approaches vary from one chiropractor to the next. While some
employ “thrust-type” procedures that lead to cracking or popping
sounds and sensations, others use gentler methods to manipulate
the bones of the spine, Schneider says. Still others treat patients
using instruments that deliver small electric pulses or light
therapy. Some use X-ray and other imaging technologies, while
others mostly eschew these tools.

This mosaic of treatment approaches can make it hard to pin down


chiropractic’s risks and benefits. Perry says thrust-type procedures
may come with some additional risks. And the Mayo
Clinic reports that, in rare cases, chiropractic adjustment has been
associated with spinal disk injuries or stroke. The latter is thought
to result from trauma to the arteries of the neck caused by “high-
velocity” adjustments, although new research in the Annals of
Medicine argues that the link between spinal manipulation and
stroke is questionable.

Finally, Schneider says some chiropractors make exaggerated


claims or attempt to treat conditions without the backing of sound
science. “I know some claim they can treat autism and learning
disabilities or conditions like diabetes or high blood pressure, and
this is where a lot of the controversy comes in,” he says. “There is
not good evidence that [chiropractic] can treat non-
musculoskeletal conditions.”

To sum all this up, the evidence to date suggests


chiropractic spinal manipulation may help some people find relief
from back, neck, nerve, and headache pain. The lack of
mechanistic evidence is a sticking point for some medical doctors,
but many now endorse chiropractic methods for pain relief
because there don’t seem to be better alternatives.

“We got most of our new patients from primary care physician
referrals,” Schneider says. For people looking to give chiropractic a
try, he says asking a trusted doctor for a chiropractor
recommendation is a good tack. “There are good and bad
practitioners in any profession,” he says. All chiropractors must be
licensed, which requires a doctor of chiropractic (DC) degree. But
asking around can help you find a good one, he says.

“I think the general public has gravitated toward non-


pharmacologic and alternative treatments,” Schneider adds. “Ten
or 15 years ago, this was still considered voodoo medicine, but the
perception has changed.”

American College of Physicians issues guideline for treating nonradicular


low back pain

Treatment recommendations include massage, acupuncture, spinal


manipulation, tai chi, and yoga

Philadelphia, February 14, 2017 -- The American College of Physicians (ACP)


recommends in an evidence-based clinical practice guideline published today
in Annals of Internal Medicine that physicians and patients should treat acute or
subacute low back pain with non-drug therapies such as superficial heat,
massage, acupuncture, or spinal manipulation. If drug therapy is desired,
physicians and patients should select nonsteroidal anti-inflammatory drugs
(NSAIDs) or skeletal muscle relaxants.

Low back pain is one of the most common reasons for all physician visits in the
U.S. Most Americans have experienced low back pain. Approximately one
quarter of U.S. adults reported having low back pain lasting at least one day in
the past three months. Pain is categorized as acute (lasting less than four
weeks), subacute (lasting four to 12 weeks, and chronic (lasting more than 12
weeks).

“Physicians should reassure their patients that acute and subacute low back pain
usually improves over time regardless of treatment,” said Nitin S. Damle, MD,
MS, MACP, president, ACP. “Physicians should avoid prescribing unnecessary
tests and costly and potentially harmful drugs, especially narcotics, for these
patients.”

The evidence showed that acetaminophen was not effective at improving pain
outcomes versus placebo. Low-quality evidence showed that systemic steroids
were not effective in treating acute or subacute low back pain.

For patients with chronic low back pain, ACP recommends that physicians and
patients initially select non-drug therapy with exercise, multidisciplinary
rehabilitation, acupuncture, mindfulness-based stress reduction, tai chi, yoga,
motor control exercise (MCE), progressive relaxation, electromyography
biofeedback, low level laser therapy, operant therapy, cognitive behavioral
therapy, or spinal manipulation.

“For the treatment of chronic low back pain, physicians should select therapies
that have the fewest harms and costs, since there were no clear comparative
advantages for most treatments compared to one another,” Dr. Damle said.
“Physicians should remind their patients that any of the recommended physical
therapies should be administered by providers with appropriate training.”

For patients with chronic low back pain who have had an inadequate response to
non-drug therapy, ACP recommends that physicians and patients consider
treatment with NSAIDs as first line therapy; or tramadol or duloxetine as second
line therapy. Physicians should only consider opioids as an option in patients
who have failed the aforementioned treatments and only if the potential benefits
outweigh the risks for individual patients and after a discussion of known risks
and realistic benefits with patients.

“Physicians should consider opioids as a last option for treatment and only in
patients who have failed other therapies, as they are associated with substantial
harms, including the risk of addiction or accidental overdose,” said Dr. Damle.

“Noninvasive Treatments for Acute, Subacute, and Chronic Low Back Pain” is
based on a systematic review of randomized controlled trials and systematic
reviews published on noninvasive pharmacological and non-pharmacological
treatments of nonradicular low back pain. Clinical outcomes evaluated included
reduction or elimination of low back pain, improvement in back-specific and
overall function, improvement in health-related quality of life, reduction in work
disability/return to work, global improvement, number of back pain episodes or
time between episodes, patient satisfaction, and adverse effects.

The evidence was insufficient or lacking to determine treatments for radicular low
back pain. The evidence also was insufficient for most physical modalities and for
which patients are likely to benefit from which specific therapy. The guideline
does not address topical therapies or epidural injection therapies.

ACP’s clinical practice guidelines are developed through a rigorous


process based on an extensive review of the highest quality evidence available,
including randomized control trials and data from observational studies. ACP also
identifies gaps in evidence and direction for future research through its guidelines
development process.

ACP’s previous recommendations for treating low back pain were published in
“Diagnosis and Treatment of Low Back Pain: A Joint Clinical Practice Guideline
from the American College of Physicians and the American Pain Society” in
2007. Some evidence has changed since the 2007 guideline and supporting
evidence reviews. The 2007 guideline did not assess mindfulness-based stress
reduction, MCE, taping, or tai chi.
